12581949Consumer: [redacted]on behalf of Customer: [redacted], a Minor Facility: Platte County Memorial Hospital Account No: [redacted] DOS: [redacted]Sir or Madam:We received the complaint brought forth by [redacted] on behalf of [redacted], a minor and the above noted account.We appreciate the opportunity to address these concerns and after a thorough review reached out to Mr. [redacted].  We were unable to reach him, but did leave two separate voicemails, with our contact information.  We will follow this with a letter that will include details of the following information and actions taken, based on account review.    First and foremost apologize for Mr. [redacted] for the less than favorable experience with Banner Health. According to our records and account details, on August 10, 2016, we received payment and an explanation of benefits (EOB) from Mr. [redacted]’s health insurer, which triggered the statement cycle to Amber [redacted], the guarantor listed on that account at that time, for the balance due on account. This amount is patient responsibility based on contractual agreements between Mr. [redacted] and his insurance company and included on EOBs sent to us with their payment.    The first statements was mailed to address we had on file, [redacted]3 and returned to Banner Health by the post office.  On September 30, 2016, the guarantor was changed to [redacted] with an address of [redacted].  The second and third statements were mailed to this address and returned as invalid address.  The account was referred out to collections on November 22, 2016 after the final statement was sent and returned. Meanwhile attempts to contact him for address updates failed.  In order to move forward and rectify this matter for [redacted], we’ve contacted [redacted]y, the collection agency and had [redacted]’s account returned.  In order to proceed with resolution, Banner Health needs to discuss payment options for the balance due with Mr. [redacted].  As mentioned a letter with detailed information and balance due, will be sent to Mr. [redacted] to the address noted in the complaint.  We hope that these preliminary steps to resolves are satisfactory to Mr. [redacted] and again apologize for this situation and look forward to speaking to Mr. [redacted] and hope to bring this to conclusion.    Please feel free to contact us with any further questions at [redacted].   Sincerely,BANNER HEALTHAnna Marie R[redacted], Mgr.Patient Financial Services Medical Center Wyoming[redacted]
