If they need a HIPPA release form, they need to mail one to me to sign and send back. Again, they are trying to stall on returning my money to me.
They need to mail this form they are requesting to me immediately and I will sign and send back. I live 4 hours away from there and cannot drive all the way there just to correct a mistake that they are responsible for.
