Review: On Monday, May **, 2013, I was contacted on my cell phone by a billing director named [redacted] working for the Polyexna / Hercules Medical PC at [redacted]. At this time, [redacted] informed me that I had an overdue deductible payment to the facility for a total of $496.00 for procedures performed on 01/**/2011 a year and a half after the procedure date. [redacted] stated that the facility had tried contacting me numerous times via mail to collect the payment due, however no bill was ever received at my house. I told [redacted] that I was going to go into the office to personally resolve this issue and to discuss the deductible payment, as I did not believe that these charges should be been incurred.I went into the facility the same day at 5:00pm and spoke with the physician on duty, [redacted], about the expense of this bill. After not being able to find my original deductible payment bill, the facility presented me with a copy. [redacted] suggested that I make an appointment for a physical at the facility and that, by doing so, the cost of my overdue deductible payment would be waived. I made an appointment that day due to his suggestion, but felt uncomfortable with the practices of the facility and later cancelled my appointment. I spoke with [redacted] about my cancellation, and told her that I would try to make installment payments towards the $496.00 bill. In July, [redacted] called me once again to remind me of making a payment that month. I had been going through a troublesome family crisis which caused finances to be tighter than normal, not allowing me to make a payment at that time. I had told [redacted] to proceed however she would have to with the billing process.On Saturday, August **, 2013, I received a letter from your collection agency, the Bureau of Accounts Control, seeking a payment now of $945.00 - $500.00 more than the original bill. As a single, low-income parent of two children, I cannot afford the original payment amount of $495.00, nor the increased payment of $945.00. Im looking to get my deductible payment waived, as the facility is unable to present me with the original proof of deductible payment, and furthermore neglected to give me timely notice of the deductible, if it actually was due.There are numerous articles listed online suggesting the malpractices of the team at Polyexna / Hercules Medical, citing [redacted] by name for unethical medical practices and patient scamming.Desired Settlement: I would like to be able to pay small installments to the total amount of $496.00, if necessary, and to no longer be held responsible for the unethical increased amount of $945.00. I would also like there to be a formal investigation into the business and medical practices of the Polyxena Medical P.C, as to avoid further patient fraud in the future.

Gentlemen:

Again I must stress to you that the patient's complaint has nothing to do with Hercules Medical, which is a company entirely separate from Polyxena. The patient presents as "evidence" a letter from Polyxena. Why do you persist in writing to Hercules?

I will accept resolution of the dispute as follows:

1. Revdex.com writes a letter to the patient and educates her that the reason the billing went up from $496 is that once she defaulted on her payment schedule we had to go to a collection agency that charges us 30%. She was advised previously of that in the letter that she presents as evidence, but perhaps coming from you she will pay attention.

2. The patient submits a letter to Revdex.com withdrawing her complaint against Hercules and states that she understands that there was no basis for sending it in the first place. She must also acknowledge that she will make payment of $50/month until the $496 is paid, but that if she defaults again, the entire bill of $955.87 will become due and collection will be reinstituted.

If these conditions are met Polyxena will accept the patient's proposal, and we ourselves will pay the $150 fee to the collection agency. Please understand that we are willing to accept this but only if the matter is put finally and completely behind us. This has been a colossal waste of time.

It is gratifying that [redacted] has accepted the offer that was made to her before we spent money on a collection service and before she wasted everyone's time with her frivolous complaint about us.

If [redacted] will now write to you withdrawing with finality her complaint against Polyxena and enunciating that there was no basis for making the complaint in the first place, we will accept her payment schedule and take her out of collection.
