Mr*** was offered a consultation and evaluation, which included posture screen, stress
test and spinal electromyographyThis was offered as a complimentary service to determine if he had a problem that could possibly be helped through Chiropractic care
After complimentary evaluation, Dr*** determined that Mr*** had a problem and he needed x-rays of his spineThe cost is $for each area of his spine; Cervical, Thoracic and LumbarDr*** offered Mr.*** a discount for his x-rays, if he wanted to continue
Mr*** agreed to go beyond his complementary evaluation and we offered him the X-rays at a discount of $instead of $
We scheduled Mr*** an appointment for a report of his findings on 6-29-and he canceled and said he could not reschedule until after 7-29-He later canceled that appointment, after we called him and left messages on 8-17-and 8-25-and never heard back from him
At that time we sent him a bill for his outstanding balance for the x-ray's of $
Mr *** received and agreed to everything that he was charged, prior to providing the servicesOur purpose is to help people with their health problems and we don't guarantee their insurance will cover
Mr*** was to pay his balance on June 29, at the time of his report of findingsHe canceled and never rescheduled, so we sent a bill for $on September 1, and Dec 1, After having an outstanding bill for months we sent him to collections on January 16,

Other than stating dates and times, which I do not question, the doctor (or his office staff on his behalf), did not answer the issues of my complaint
1) I agreed to place a $deposit toward the x-rays as a result of being pressured for an instant answer when he and his assistant pressured me for my permissionI issued the deposit because the claim would be submitted to my insuranceI fully understand that no doctor can guarantee that an insurance company will provide coverageIf this "doctor" never followed through with submitting my claim, then he guarantees they will not! According to his office, they never intended to submit it because they "knew it would not be accepted." As I said before, if I had been told that, I would never have given permssion in the first place
2) The only reason I went to see him was because of his enticement to discuss my Diabetic conditionI firmly believe the doctor's main intent was to convert me (and others from his Diabetes Seminar) into an ongoing chiropractic patient
3) In the response, he has neglected to accept, oreven address my proposed resolution to this matter
