Please note that we received the paperwork on *** a little late and this is why I am replying now, sorry for any inconvenience. We agreed to cancel her membership per the Day Clause because there was no proper management at the club when
she need it, as we were in between billing managers. I went ahead and left her a message today to see if she did in fact want a month to month contract, in which I would reinstate and change

To whom this may concern, T o whom this may concern, Please note that [redacted] came into our facility on December 31, 2013 and tried cancelling her gym membership due to a medical reason. The proof that she supplied to us was a work-out schedule from [redacted] that stated certain workouts she should be doing. The billing Manager ([redacted]) explained to her in order to cancel for a medical reason you need a medical doctors note stating permanent disability to cancel immediately with a 30 day notice. [redacted] also explained to her that if she contacts her doctor and get a note stating she is unable to workout she can place her on a 6 month freeze without payments. She refused to call the doctor and walked out. We will be happy to place [redacted] on a freeze if she gets us a note from her doctor stating she cannot workout at this time or we will cancel her membership with a 30 day clause if she gets us an MD note stating permanent disability.
