Thank you for the opportunity to respond to this issue.  We acknowledge receipt of the letters sent by Mr. [redacted].  The billings that were...

sent to Mr. [redacted] were not in response to his letters, they were sent to him according to our monthly billing and collections process.  In response to Mr. [redacted] first letter, our records show that we telephoned him on 5/23/11 and there was no answer or answering machine to leave a message on.  On 6/6/11 we called Mr. [redacted] at his work place and left a message for him to return our call.  On 6/20/11 we received a phone message from Mr. [redacted] to send him an itemized bill.  An itemized bill was mailed to Mr. [redacted] on 6/20/11.  On 8/24/11 we received a second letter from Mr. [redacted].  At that time the letter was referred to our Director of Patient Services.  Ms. [redacted] telephoned
 
Mr. [redacted] on 9/1/11, 9/2/11 and 9/17/11 and was not able to make contact with him.  On 9/26/11 a letter was sent to Mr. [redacted] from Ms. [redacted] asking for him to give us another number to reach him at.  She explained she had attempted to reach him on several occasions by phone to answer and clarify questions he had on the bill.
 
To my knowledge there was no response from Mr. [redacted].  On 7/11/12 Ms. [redacted] attempted another phone call to Mr. [redacted], again with no answer.  On 7/16/12 the account was referred to our collection agency in the amount of $626.82.  Mr. [redacted] has questions that remain unanswered and I would like to suggest that he call the hospital and set up an appointment time and date for Ms. [redacted] to call him so that she can address his questions and both will be assured they will be available to accomplish this.  I would be happy to assist Mr. [redacted] in making that appointment.  If Mr. [redacted] would like to speak with Ms. [redacted] in person, that also can be arranged.  I can be reached at [redacted] and am looking forward to hearing from Mr. [redacted].  If I can help in any other way, please let me know.
