Preferred Home Care

Review: Agency was hired to provide in home care for my mother while she was on hospice. When we hired them we were assured by [redacted] that the agency had a current business license, liability insurance and other requirements of long term insurance so we could be reimbursed for the service. I was informed by Ms. [redacted] and [redacted] Long Term care insurance that we should pay the bills and would be reimbursed by [redacted] as long as my mom qualified-which she did. [redacted] originally requested Preferred Home Care's paperwork and information on 12/18/2013 by mail. I received a call from [redacted] on 1/3/2014 that they had not received a response from Preferred Home care so I called Ms. [redacted] and gave her the information that [redacted] needed and their contact information.. She told me she would take care of it right away. The matter was unresolved on 1/20 and again I talked to Ms. [redacted] who assured me she would take care of it that day. On 1/20 [redacted] also faxed a second request to Ms [redacted] had still not received the information on 2/5/2014 so I called Ms. [redacted] again. That phone call and subsequent phone calls and text messages have gone unanswered or returned. A letter and fax stating the issue, what [redacted] needed and to send me the necessary forms as well was sent 2/12/2014. USPS tracking confirms the letter was delivered 2/14/2014. As of February 20th I have still not heard back from Ms. [redacted] or anyone else at Preferred Home Care.Desired Settlement: I need Ms. [redacted], or someone at Preferred Home Care, to provide to [redacted] with:1) Business and State License to provide home Care2) Professional Liability Insurance Information3) Billing and Daily Visit Notes4) Staff Training and Supervision Records5) Home Health Agency StatmentI would also like a copy of this information for my own records. If the information cannot be provided I want the issue reported to the appropriate authorities.
