To whom it may concern: When [redacted] first came to our office on November 10, 2014, she gave our front office staff her United Health insurance cardShe did not let our front office know that she had another insurance called VSPNowhere on her United Health insurance card does it state that she has VSPUnfortunately many patients who have United Health insurance do not also have the VSP insuranceSince [redacted] states that she also has the VSP insurance, we will gladly submit this tomorrow for her Warm Regards, [redacted] ***

My name is *** *** O.Dand I am the doctor that examined *** On July 10th, I did an examination of *** ***, and as a courtesy I gave him a trial set of Contact Lenses to wear out of the office He complained things seemed blurry but when I tested his vision back
in the exam room his vision seemed good and *** told me that the lenses had adjusted and he was seeing better When he left the room I told him and his father that if he continued to have problems to come back for a follow up visit *** did come back a few days later with his mother and I wound up repeating the entire refraction and I did in fact change the powers for one of his eyes His vision was 20/in each eye and 20/with both eyes together which is above average vision So I revised his prescription and he left with his mother A few days later the ***'s mother called our office and left a message that said that later that evening ***' complained his vision had gotten blurry and that he wanted a refund The next day I called them back and spoke with ***'s father He said they had gone to another office for a second opinion and had gotten a different prescription than I had and he wanted a refund I provided services to *** twice and I know he saw well when he left my office I told his father that if they came and we compared the prescriptions and found a "significant" difference in the prescriptions I would give him a full refund She showed me the prescription he'd gotten from DrJennifer Johnson The Prescriptions were virtually identical There are numbers in a prescription "sphere", "cylinder" and "axis" It would be very rare for prescriptions to be identical, but in this case the prescriptions were virtually identical There was a +Diopter difference in one eye The Prescriptions were 98% identical I showed the mother the numbers and explained the the prescriptions were for all intents and purposes the same She claimed that I told her husband that if the prescriptions were not "exactly" the same, that I would refund their money I never said that Most optometrists wouldn't even offer a refund after providing demonstrably quality service to the patient It was not true I never told the patient's father I would give a refund if the prescriptions were not "exactly" the same I'd seen *** before, and he was wearing the same brand of lenses he'd worn successfully the exam before And the lenses he left with the second time were almost identical the the Prescription written by the doctor they'd seen for their second opinion I asked if the lenses the second doctor had provided him made him see better, so I compare the vision The mother said the second doctor had not given them lenses but they would be getting trials soon So *** had not even worn contact lenses with the "new" prescription I had provided *** with essential contact lens examinations, and then spent minutes explaining to the mother and *** how the prescriptions were the essentially the same I went over and above on the services I provided that family and, in my opinion the father either lied or miscommunicated to the mother the situation and I did not give them a refund I did not hear back from them and don't know if *** ever even found contacts that worked for him So considering the father's actions, the exams, and time spent explaining to the mother I feel I went far above and beyond to provide good services but they were not reasonable, not understanding, or just wanted to complain I stand by my decision
