To whom it may concern,I am writing in response to your letter dated 06/24/concerning a complaint you received from Mr [redacted] ***.Lydia M [redacted] , CRNP, saw Mr [redacted] for a follow up on 5/14/Lydia referred Mr [redacted] to an orthopedic surgeon for evaluationHe was given an excuse until May 20, 2015, the date that he was scheduled to see DrB [redacted] On May 26th, Mr [redacted] dropped paperwork off at the office to fill out for his employmentThis paperwork addressed his ability to workHe was advised that his orthopedic surgeon should fill these forms out but the patient stated that they refused.The patient's wife presented to the office on June 9th, and stated that the patient did not want the orthopedic surgeon to fill the paperwork out because they charged too much.After further investigation, it was determined that the orthopedic surgeon was never given the paperworkPer [redacted] at DrB [redacted] 's office, they never told the patient to have his family physician fill out the paperwork.Since we were not the treating physician, the forms that Mr [redacted] is referring to needed to be filled out by DrLance B [redacted] The patient was left messages from our Operations Supervisor to discuss the situation and to date, he has not returned her calls.Sincerely, [redacted] Operations Manager Premier Medical Associates

For your reference, details of the offer I reviewed appear below.Reference # ***To whom it may concern,in response to what Premier Medical Associates had stated in their response dated July 2, 2015.When I saw DrLance B*** for my dislocated thumb I had asked the Doctor himself while in the examination room in the presence of my wife about what to do about forms for my employer at which time DrB*** told us that my P.C.Pwas responsible for any forms.So per the information that the doctor provided us, I took the forms to Premier Medical AssociatesI paid for the forms upon dropping them off.I called to check on those forms later that week and was told that forms take 7-days.On June 9th my wife did not say that the orthopedic charged too much for the formsShe informed them that we had already paid for Premier Medical Associates to complete the forms and due to the fact that they had not been completed that I had not received a paycheck in a few weeks and that we were not even able to pay for the insulin that she needs desperately during her pregnancy.The reason that the orthopedic surgeons' office never received the paperwork was because DrB*** had told my wife and myself that they did not complete forms and that Premier Medical Associates was responsible for completing them.Furthermore I did return the operations managers call twice actually and never received a return call from herJust another example of how little they actually care about their patientsSincerely,*** *** ***

Love my doctor, but PMA is the worst practice group I have ever seen in my 75 years. Beginning 12.6.18 I began trying to get in touch with my Primary using PMA's patients portal. Message is not on the portal. Then faxed him and no response. The typed and hand delivered a message to his administrative assistant to give him. No response. Called again and left message with his AA, got a call back from his PA, who said he would check my file and call back. That was before Christmas and the first call back I've received was this morning from a very impolite female who said I would have to reschedule because I didn't get a blood test. After a hostile conversation, I said I would be there for my appointment, and would raise public hell if I was denied access to my Primary. After talking to her supervisor, I will see him in an hour. At this point I think PMA is hazardous to my health. As a retired attorney, I document my complaints and am anxious to see what PMA's response is. I hope they know that the truth is a defense to defamation, and only respond with the truth.

Review: I was in a car accident on May 10th. I was seen in the office as a follow up to the E.R. by Lydia, the nurse practioner. I was given a medical excuse by her to not return to work. During this time I was also seen by an orthopedic specialist who scheduled an MRI. All copies of my medical records including the MRI results were given to Premier Medical. Because of this accident I was on short term disability with my employer. They required documents to be filled out. I paid premier medical to handle the paper work but they never submitted anything. Both myself and my wife have called the office on numerous occasions but would never receive a call back. It is now June 8th, almost a month later, still they have failed to fill anything out claiming it is not their job. They glad took our money though. Because they refuse to fill out the paperwork, or refund the charge to do so, I have not received a paycheck from my employer. My wife is pregnant and diabetic and we cannot pick up her medicine because of this. We have called the office non stop and are told Lydia will take care of it tomorrow, it'seems been almost a month of this and still nothingDesired Settlement: This complaint to be published so others can be aware of the shady practices of premier medical. They do not want to do their job or return phone calls

Review: this company out grown them self, I have added a list of issues that have happened with in the last year,

- medication was not communicate between manager, doctor and pharmacy, took over a month to get my meds to the correct amount discussed in appointment.( I went over 2 weeks with no meds)

- waited over 1 1/2 past my apt. time to find out even with a confirmation call my apt was not in there system.

-Charged for missed apts,

-->> one I clearly talked to the front office and informed I did not have insurance so I would not be coming -Still charged

--->> charge 2 answered recording told to press 2 if I could not make it yet again I did , still charged!

-threatened by manger to kick me out of practice ( for missed Apts)

-disrespected lied to as far as who was spoken to with issues

- charged for physical I DID NOT receive!!

- waiting over an hour in waiting room past apt, adding 30-1 hour in room for doctor.

- returning calls taking over a week (untill I call again)

- information regarding issues NOT documented well in the system!

- Doctor having NO idea about medication issues when I was told he was spoken to.Desired Settlement: charges taken away. better trained staff

Review: I am being billed for a co payment that was paid by my medical carrier---my carrier fax over proof of payment to premier several times to no avail.My co payment is $10 and I paid the ten dollars at the time of my office visit. My medical carrier was first saying that my co payment was $20 because the coding the doctors office was using was when my visit was submitted for payment. I was treated by my PCP for the office visit in question co payment $10. My carrier [redacted] made the correction on their end and issued a payment to Premier on May 2015. Premier has yet to update their system showing my carrier added payment of $9.80. This has been going since March2015 for an office visit of 2/26/15. Every month I receive a bill from Premier so I call Premier and I call [redacted] for proof of payment to be fax over. Today I received a bill stating they are going to turn this over to a collection agency with in ten days.Desired Settlement: correction of their system
