I went to Dr. *** of Premier Orthopedics, for both of my knees. Left knee is for workers compensation and the right knee is bad. They told me I had to have separate days, so they would not mix them up between *** and workers compensation.
I went on August 21, 2019 for the right knee which is *** To get a shot in that knee. They said they had to cancel me, because I had different phone numbers. I said that was OK, but then a person came out to see me and told me the Doctor can make room for me that day, so I waited for a couple hours. They took me back, he bent my knee up and down, but did not take an x-ray. The Doctor said they would have to contact *** to get the shot approved. *** contacted me and said they refused the shot, because the Doctor did not take the x-ray.
Went on October 17, 2019 for the left knee which is Workers compensation in Ohio. To get a shot in that left knee also. They took the x-rays, but sent it to *** instead of Workers compensation.
I informed *** that the bill I got showing what they paid was for workers compensation.
I then called Premier Orthopedics and explained to them about the mix up. I called workers comp. to let them know also. Workers comp had the Ombudsman call them, but no one had called me back.
I was informed by Ombudsman that Dr. *** was quitting and the other doctors did not have time to see me.
I now have a new doctor for my right knee, but he does not take Ohio workers compensation for my left knee.
Now I will probably loss my workers compensation, because of Premier Orthopedics. How long does it take for them to give me a shot in my left knee for workers compensation.
Finish the job Give me the shot in my left knee for workers compensation.
Chart was investigated. This patient was seen on 8/21/19 for the right knee and *** was ordered and sent for pre auth to *** Denial was received on 9/9/19 stating no x-ray done. It was resent on 9/11/19 with copy of x-ray report from previous visit. There is no further correspondence in the chart regarding denial or approval. I am not sure why. We were having employee turnover at that time so that may be the cause. That is no excuse and I do apologize. Patient was seen again on 10/17/19 for the left knee, and the note does say we will preauthorize *** but there was no written order found in the chart, and no notes indicating that preauthorization was started, and our referral person did not find this patient's name in her injection log under date of service 10/17/19. I assume she did not receive the order from the doctor. I looked at the patient's account in *** and the 10/17/19 date of service was billed to *** and was corrected in February and sent to the W/C carrier. We do not do the billing here in the office. There is a note in the patient's account that states the left knee is work comp. Again I do apologize for these incidents. We can go ahead and get the *** preauthorized with the work comp carrier for the left knee and we will schedule the patient for the injection with Dr *** NP, if that is her wish. We did send out letters to Dr *** patients that he was leaving. I do not know why she did not get one.

All I was wanting was the shot in my left knee. I will contact the Doctor and make an appointment to receive the shot, Thank You.
