Complaint: ***I am rejecting this response because: That does not address the flaws in the service I have been provided and the lack of professional care and concern needed by
patients. There is always a break down in the primary health group completing their required duties that they have charged for and what you went in to have addressed! That response does not address why they could not complete my blood tests without me having to come in and have blood drawn again cause of their negligence to submit the required tests to the lab that they should have done!I was subsequently charged for the blood to be drawn and then would have had to gone back in to be charged again when it was their mistake and lack of employees to do the required requests to the lab. Also their answer does not address why patients have to continually phone them to get things done like my prescription was not sent to the pharmacy without me having to call them again. Some one at this care facility has not been completing their job! I was charged and office visit as I went in for my blood pressure medicine to be renewed and they said I had to have a blood test and would have to see the provider again. I was just in for a CDL physical and a yearly physical. Why would the provider not have addressed this at these two things. I was billed for a CDL physical. And a annual physical charge was sent to my insurance companySeveral items are duplicated in each physical but they still charge you full price for both. There should be a price break! Or they should have taken my blood pressure twiceWas charged for it. UA test twice I was charged for it, vision test. When u have both done on the same day there has to be a reduced rate. But no they nail you full price for both and several items are duplicated and they only have to do them once!!!!!!! I say that is fraud but insurance don't cover a CDL physical you pay cash. totally wrong! Their answer does not address their screw up for not getting the right lab test completed as that was what my appointment was forA LDL test for my blood pressure medicine. Then when you call them on their flaws they suspend your patient treatment ! What an easy out for not doing their job!

Mr. [redacted] was seen in our clinic on 7/20/16 and 7/26/17.On 7/20/16, he incurred charges of $229.00. $11.85 was written off for an insurancecontractual adjustment. He paid $30.00 on 12/1/16 and $35.00 on 1/20/17. He had a balanceof $152.15.On 7/26/16, he incurred charges of $184.00. $25.24 was...

He has not paid at all on this visit.He received statements on 8/12/16, 9/12/16, 10/17/16, 1/18/16, 2/19/16, 1/19/17, 2/20/17,3/23/17 and 4/24/17 before going to collections. I think we've been more than generous.Any interaction we've had with Mr. [redacted] regarding this issue have been inappropriate atbest. The account is now in the hands of CBI, our collection agency. This matter has been resolved with the consumer.

Complaint: [redacted]I am rejecting this response because:They have avoided the main issue of failure to submit the right lab sample for the right tests. No mention has been made on this very important patient safety issue. The only reason the prescription issue was solved was my wife called and someone finally seen the issue was handled. Again another shortfall by a medical provider! But the issue of not completeing the right requests for the right lab tests and needing another sample for their failure to order the correct test is not addressed. Evidence that they think they are invinsible and above patient safety!. To resolve the issue they terminate patient relayionship. should also be noted I requested from the office adminstratoe my medical records and she told me it would be 7-10 days! Guess what months later I had to call again! Another example of short falls of this clinic and their lack of concern for patients. You always have to call and ask numerous times for anything. Even when they should of called me on my lab results I never heard from them till I called them!. I plan on submitting a complaint to the American medical Association and to the Attorney general for an investigation into clinic operation and patient safety. I am putting together a packet for that office as I type this!
