Review: Prime Dental Group (PDG) sent me a bill for invalid services. The dentist, Dr. L**, recommended a treatment plan to which I agr**d. I was covered by dual insurance plans. The cost of these procedures is known to the dental office and detailed and agr**d to in the f** schedules of the insurers. According to the "ADA Dental Patient Bill of Rights and Responsibilities Statement", patient's right number 8 is "You have a right to know in advance the expected cost of treatment". I never received a quote or estimated cost statement from PDG for the procedures in question. Dr. L** of PDG is an active member of the ADA and the CDA. I was informed by PDG front office staff that all my treatment plan costs would be covered by my 2 dental plans. I was not asked to pay any copayment, deductible, retainer or any other type of advance payment. Each time I visited PDG for regular cleanings, I asked if I would owe anything. Each time I told "No". About 7 months after my last visit, out of the blue, I received a bill from PDG for over $350. I now refer ADA Patient rights number 9 that states "You have a right to accept, defer or decline any part of your treatment recommendations." It is impossible to make a logical decision about treatment recommendations if you do not know what the expected cost of the treatment is in advance of the procedure. If treatments were performed that were not properly claimed, supported by required documentation, or considered valid by the dental insurer, that is beyond my control and not my responsibility. I was never provided a quote of estimated costs prior to, at the time of, or in fact up to 7 months after treatment. It is because of this that I believe these charges are invalid and unethical.Desired Settlement: I ask to have PDG:1. Discharge this entire erroneous billing amount of $446.11 which includes $398.44 in principal and $47.67 in interest/late payment charges. 2. Cease & desist any and all collection activities against me.3. Remove any negative credit actions that may have been reported to a credit reporting agency.
