I disputing a charge in the amount of $ for a vitamin D test
Issue: I went to La Verne Medical Group and saw Dr*** on February 22,for a routine checkupAs a part of the doctors panel of tests, he ordered a Vitamin D test (This test was ordered without my knowledge or permission)The Vitamin D test was not covered by my insurance company (Blue Cross) because the test was not considered medically necessaryI have since received an explanation of benefits stating that the test was not covered from Blue Cross and bills from Primex (company that performed the test) & LABCORP (biller for Primex) for payment of $I do not believe that I owe this amount as I did not authorize the testThe doctor and La Verne medial group should have received approval from my insurance company prior to ordering the testIf the approval was not granted by the insurance company, then the test should not have been performed without first seeking my permission

Primex were apparently sent bloodwork from my urgent care, didn't charge my insurance but instead started sending me bills for $to an address I haven't lived at for yearsNeighbor finally gave me one that arrived recently threatening to turn said bill over to a collections agencyCalled in and the manager refused to clear it up herself, stating that I need to pay for it then submit the claim insuranceTheir workers are all rude, irresponsible fools
