Mrs*** *** presented at my office on January 12, 2015, with a complaint oflower back pain with co-morbidities of neck pain, left arm pain and bilateral knee pain,arthritis and goutThe primary complaint had been present "for years", and the lastcouple of months she decides to get
something done due to the pain.She had been taking ibuprofen and she also had seen an orthopedic surgeon who told hershe had bulging discsHe prescribed medication that she could not take due to anallergic reaction.An evaluation was performed and recommendations were madeShe was also informedof other treatment optionsShe was given materials describing the recommendedtherapies and encouraged her to research them to become more familiar with themOnthe second visit she was given a complete session of all modalities at no charge so shecould experience the recommendations before deciding to pursue the course of treatment.Upon acceptance of the recommendations Mrs*** was given a form with all feesand a notice that acceptance of the recommendations did not imply or guaranteea cureShe signed the form acknowledging the caveat.She began care on January 15, She showed improvement within weeks reportingdecreased pain January We discussed light exercise at that visitShe continued slowimprovement until February when she caught her foot on a chair leg when leaving theoffice, and apparently twisted her lower backShe did not inform me at the time, but toldme on her next visit February By February she was improved with less back andleft knee painShe continued to show slow improvement through March 23, Shewas seen an additional visitsThe additional visits with multiple procedures totaled$Mrs *** was not charged for these additional sessions.She was discharged April At that time she had soreness in the lower back muscles.She was instructed to continue with alternation heat/ice, avoid unilateral movements suchas vacuuming, and encourage to do light exercise such as walking.No financial adjustments will be made.Sincerely, Steven C Riggleman, DC
