been reviewed by our company's dental director.  Many times a tooth may
develop an infection at the root tip that requires root canal treatment to
resolve the infection.  This procedure is...

often less expensive than
removal of the tooth and replacement with a denture of fixed bridgework. 
Every patient's needs are different.  In your case, you contacted the
office on September 1 expressing pain in the upper left premolars.  You
were seen the next day when treatment began.  A general
dentist opened the tooth and encountered obstruction in the root chamber or the
canal beyond.  She placed medication inside the tooth and filled the
access to the root chamber.  You were then referred to a doctor who
specializes in the negotiation of difficult or obstructed canals, and has a
superior success rate in the preservation of infected dentition.  In
reviewing your clinical record, our supervising dentist can find nothing
incorrect in the treatment rendered by either treating doctor at [redacted]
Dentistry.  I'm sorry to hear that you feel you were lied to about the
need for the procedure.  Your complaint does not mention ongoing
discomfort with this tooth, therefore one must assume that you have received a
benefit from the procedure.The
question arises as to why your previous dentist, who originally recommended the
root canal, did not perform the procedure himself, but instead referred you to
another dentist.  Secondly, how are you able to contest the diagnosis of a
calcified canal and the subsequent need for endodontic treatment?In
a review of the billing in your case, it appears one error has been
made.  In the attempt to resolve your pain as quickly as possible, you
were scheduled with the Endodontist in the Elgin office instead of the
Endodontist in the participating office in Arlington Heights.  While [redacted] in Elgin is participating with all of our other insurance contracts,
your insurance company is still processing his paperwork.  Our error is
that you were referred to a provider who, at this date, is out-of-network. 
For that reason alone, a refund of $467 will be issued to you.  Of the
$658 paid on September 8, $4 was balance from 9/2/2015, $5 a balance from
9/3/2015 and $5 from 9/8/2015.  This $14 difference is your responsibility
outside the issue of the out-of-network referral, according to your [redacted] plan. Please allow 14 days for our accounting department to issue
the refund check.
