Patient, [redacted] called to make an initial appointment at [redacted] Dentistry, in Evanston, IL and was given one for February 7, 2017, with Dr [redacted] On that initial phone call, the patient was informed that a cleaning could not be guaranteed at the first appointment, as that would be determined by the dentistIt was explained to Ms [redacted] that at the first appointment all necessary x-rays would be taken, a complete oral exam given and a comprehensive treatment plan to address any problems would be discussedMs [redacted] came in on February 7, for her appointment, x-rays were taken and an oral exam completedWhile in the chair Ms [redacted] requested teeth whitening and was told she would need to have a cleaning completed firstAt that time she insisted on getting her cleaning that same dayThe staff made every effort to accommodate Ms [redacted] request for that day and told her she could come back at 1:for that appointmentMs [redacted] insisted she be seen at 1:30, not 1:When the staff relayed the fact that the earliest they could get her back in that same day would be 1:45, Ms [redacted] became very confrontational and began using profanity, complaining and using insults toward the staff which made everyone uncomfortable including other patients in the waiting roomAt this time Ms [redacted] was asked to calm down and because she didn't, was asked to leave the officeSent on: 2/20/5:16:PM

Ms. [redacted] came in on February 7, 2017 for her appointment, x-rays were taken and an oral exam completed. While in the chair Ms. [redacted] requested teeth whitening and was told she would need to have a cleaning completed first. At that time she insisted on getting her cleaning that same day.
