[redacted] , I have nothing as far as statementI never got a single thing from them till February when I called them again When I did find out about a bill they never asked my about an address confirmation at allThey just sent it And that was the first I ever saw a bill So its not possible they sent a statement that many times to the same address and I never got one [redacted]

Dear [redacted] , I am responding to the letter of complaint with ID # [redacted] . - Pt was seen on August 4, 2014 by our doctor for her exam and extensive testing to confirm the diagnosis that he made. The patient never came back for further testing that was needed to follow up on... her condition. She never made an attempt to speak with the doctor to verbalize any of her concerns about her diagnosis. She made the decision to seek out a second opinion and is now requesting her medical records. The patient has a balance due of $169,69. We have sent the patient statements with this balance due on 10\{\14, 11\3\14, 12\22\14 and 1\22\15. As of today the balance remains unpaid. I am enclosing a copy of the invoice as per your request. The patient was informed that we cannot release the records until her bill is paid. We have all of her family's records with none of them being lost as she stated in her complaint. We also have the signed release and doctor she would like them faxed, Once her bill is paid we will be happy to fax the records to the doctor. The patient also stated in her complaint that she believes we are incompetent due to the doctor making a diagnosis she does not have and we are just trying to take money for any diagnosis that we can. The patient's condition was diagnosed based on her exam and advanced testing that the doctor performed on her. Our practice is very advanced with the services we offer to our patients that may not be offered at other optometrists offices. The patient is welcomed to speak with our doctor for more explanation if necessary. If necessary, her doctor may call our doctor for discussion, once We fax the records to him.If you should have any questions or concerns, please do no hesitate to call us. I am rejecting this response because: Per the letter from the [redacted] doctor,  It is my right to seek a second opinion.  Several test were done and many were performed by the staff not the doctor.  But I was told there was no abnormality in any of the test performed to explain high pressure, but he continually wanted to do more.  There is no hereditary reason and no visual impairment, but he insisted there was a problem.  That being said.  it is his opinion and I chose to see another opinion which is my right.  Upon seeking a second opinion,  the doctor found no reason to continue to keep testing since there was no abnormality found.  The tests are only as good as the operators taking them.  I will not go into the number of times the staff had problems working the equipment. 
 Issue number 2 which is more the problem that I have with this facility.  The staff is incompetent.  It is a flat out lie that statement were sent.  I have not change my billing address it is impossible that 4 statements went out and I did not receive a single one.  and I have called them 4 or 5 times requesting records since September and never once was I  told about a balance.  And more importantly I have an excellent credit rating and in my history with them I have never once been late on a payment, which speaks mountains to my integrity. and they should have had the decency to even make a phone call concerning a bill, which also never occurred.  I also find it immoral and unprofessional that several records relating to other patients are being held up because of a bill on another patient's account.  And these records were unavailable when another doctor called requesting information on contacts that need to be dispensed.  Resulting in this doctor go by the patients memory on the type of contacts she wore. 
My resolution to this matter.  Patients BEWARE!   The doctors rely on computers being used by uneducated staff and not there own common sense.  And that I my opinion.  I will pay the bill and I expect the records to be sent with in 24 hours.  But to future potential patients run the other way.

This is in response to the letter dated October 28, 2013 for the complaint submitted 10\16\2013 by ID [redacted].

Upon investigating this matter, the amount of $179.54 is the patient's responsibility as per his medical insurance deductible that his insurance states is owed to us for the medical testing that was completed on 7\12\13.

The following is a breakdown for that amount:

Office visit charge $95.00 Insurance allowance $79.00 Pt paid copay $20.00

Paid to us by insurance $59.00

Glaucoma exam charge $55.00 Insurance allowance $46.80

Pt owes deductible $30.54 $30.54

Paid to us by insurance $16.26

VEP testing charge $160.00 Insurance allowance $69.00

Pt owes deductible $69.00 $69.00

No Payment to us from insurance

Visual Field testing charge $95.00 Insurance allowance $80.00

Pt owes deductible $80.00 $80.00

No payment to us from insurance

$179.54 pt deductible owed to us

We confirmed with the patient's insurance company that he did receive an Explanation of Benefits with all of the above explained. Also, it does state that if he has any questions to call ###-###-####.

The patient was seen in our [redacted] Office on 7\3\13 and paid $212.00 for his glasses and office visit, which the remainder was covered by his vision plan. The above tests were completed in our [redacted] office on 7\12\13 and was billed and covered under his Medical insurance and that is where his payment for deductible for $179.54 is owed to us.
