I contracted with Cook Waterproofing and Foundation Repair to have my driveway replacedWork was completed and paid in fullWithin months I noticed there were areas on the drive where there were "pop-outs", holes where aggregate came looseI called Jeff and he assured me he would address the concern, but wanted to wait until the spring, I agreedSpring came and I re-contacted Jeff, he ask if we could wait a while as he was very busy, I agreed as I had no reason to believe he would not honor his wordBIG MISTAKE on my parthe continued to slow roll meThen he would not return my callsFinally after a year and many calls, he said he would come out and l*** at itSupposedly he inspected it, nobody saw him, and he tells me it was caused by me using rock salt on the driveThe pop-outs were noticed prior to winter, and I never used rock saltFrustrated, I contacted an independent licensed professional engineer, at my expense, to assess the condition of the concreteHis conclusion was the pop-outs were caused by air, water, mix, application and type and size of the aggregates, NOT rock saltJeff absolutely would not stand behind his workI even contacted the concrete supplier, *** Ready MixThey did come out and inspect the concrete and they furnished me the mix reports for each loadThey told me Jeff was not a regular contractor they worked with, they didn't know himThey assumed no responsibility for the concern, they definitely didn't think the was a problem with their mix, even though the engineer had concernsIt was determined by the mix reports that I had not received "fiber reinforced" concrete as stated on my contract, Jeff did not order it *** did apply a sealer to the drive at no charge, I believe to slow the deterioration downI waited too long to file my complaint with small claims court The bottom line is Jeff C*** did not supply the level of concrete that was stated on my contractHe lied when he told me he would "take care of it"He refused to stand behind his workHe can't be trusted at his word

We purchased our house back in January with assurance from Jeff Cook that the work done on the house before purchase would transfer the guarantee to usI called Jeff in February, telling him that the storage room is still leakingHe said to me that the work was not completed, they still needed to come our and regrade the dirt by the wallHe said to tey that first and see where we need to go from thereJeff Cook never came outI called several timesNow the storage room is leaking even worse and he will not fufill his guarantee
