Review: I used the Psychology Services of [redacted], MS ([redacted] Jr, MS - Uniontown, PA - Clinical Psychology)for my Act 235 exam on 11/12/13. The Act 235 paperwork (SP 8-200B) still has not made it to its designated location (Lethal Weapons Certification, [redacted] State Police, [redacted]). The exam cost $195.00 and I was given a receipt for services. The Act 235 paperwork still has not made it to its designated location to date. The Lethal Weapons Certification Office has gotten involved in my inquiry of the missing paperwork. We both have placed multiple calls to [redacted] with little to no luck. The inadequate services provided [redacted] has cost me two scheduled lethal weapons classes (December 2013/January 2014) and being hired by the [redacted] as a Security Guard because of the Act 235 paperwork not reaching its designated location. [redacted] has stated, "I have sent it twice since the exam date." He has not provided no proof to the Lethal Weapons Certification Office or to me. I am retiring from the active duty military (Army) on 1 February 2014. I started this process in October 2013. The inadequate services has delayed my hiring opportunity. I will be losing half of my current income once I retire. This Act 235 would have enable me to get hired and supplement my income. His current office currently has a For Sale Sign and always locked up. Lethal Weapons Certification Office and I can not get in touch with [redacted].Desired Settlement: I am requesting that [redacted] send the Act 235 exam paperwork within 1 week or I get a full refund (deadline of 6 January 2014). I am also requesting proof that the exam paperwork was sent as well as an inked copy of the paperwork for myself. The paper that is sent to the Lethal Weapons Certification Office has to be in original ink not a copy. I am willing to work with [redacted] with a suspense date of 6 January 2014. I the timeline for my army retirement is rapidly approaching!!!
