insurance plan. Which they would paid to the patient directly. Patient’s insurance company do not provide us details of payments (such as an explanation of benefits (EOB)).  We had put trust in the patient that the patient will turned over the checks and eob’s over to [redacted] Therapy. Not all 16 dates of service were reported to [redacted] Therapy/ PT Medical Management. Because of these reasons, we billed the patient for the full balance, minus the co-insurance payments that were received at the time of service.  It is our understanding now that all payment has been handed over to [redacted] Therapy and Mr. [redacted] account with [redacted] Therapy has been updated to reflect a zero balance.  Please contact our office at ([redacted] should you need any further information. Sincerely,[redacted]

Review: I received physical therapy services from [redacted] Therapy, [redacted] from January to April 2015. Before I started therapy, I was told by [redacted] who works in the [redacted] Therapy office, that my co-pay for each visit would be $22.50. I started and completed service and paid the co-pays after each visit. Today I received an invoice from PT Medical Management for $2,217.00 which shows my co-pays but does not show any payments by my insurance. Additionally, each visit is billed at between $173.00 to $178.00. I never received any invoice from [redacted] Therapy and, I was never told I owed any additional monies when therapy was completed in April 2015Desired Settlement: Adjust amount due to [redacted] Therapy to zero which was my understanding when service was completed in April 2015. Additionally, I believe PT Medical Management is a collection agency and I want no contact with it.
