From: [redacted] Date: Fri, Nov 20, at 3:PMSubject: FW: [redacted] Complaint ( [redacted] )To: [redacted] < [redacted] @myRevdex.com.org> [redacted] ,Below is Tibbs Paving’s response to Complaint # [redacted] To Whom it may Concern,In response to the complaint # [redacted] by [redacted] [redacted] , the Original Base Driveway was installed by Tibbs Paving for our client [redacted] in the [redacted] Subdivision in late Fall of The driveway was a two lift asphalt system for a brand New HomeTibbs Paving was asked by our client, [redacted] to return in the Spring of ( June 3) to put down the Top Coat of asphaltRight before we were scheduled to put the Top Coat down, we were asked by [redacted] to get with the New Home Owners as they wanted some additional asphalt work doneWe gave the [redacted] ’s a Cost Proposal for to install an expansion area of exactly square yardsThis was in addition to the original foot print of the drivewayThe [redacted] ’s accepted the Proposal for the additional work and we returned on June to complete the Top Coat for [redacted] and the new expansion area for the [redacted] ’sEverything was fine and there were no low spots or spots holding waterWe have yet to receive any complaints from our client, home builder [redacted] , concerning low spots or pools of waterOnce Tibbs Paving received the initial call from the [redacted] ’s concerning a low spot in there driveway that was supposedly holding water, a representative from Tibbs Paving went to meet with the client to discuss this issueHe was shown a picture of some water laying in the driveway, which we feel was taken after one of the areas heavy rains that we have had this year, however, that is not a situationTibbs Paving has since returned to view the driveway after a rain, and we feel that the water drained off of the driveway in an accepted amount of timeConsequently, we feel that there are no water issues to correctPlease remember that the [redacted] ’s complaint is not about the New expansion area that we did for them, but their complaint is about the work we did for [redacted] , who has no complaints concerning the work that was doneThank you and have a great day / David [redacted] S [redacted] V.PTibbs Paving

Initial Business Response / [redacted] (1000, 5, 2015/10/12) */ As Manager of Customer Service at Hill Physicians Medical Group I ( [redacted] S [redacted] ) received this consumer complaintAs with any member correspondence I begin researching the member issue using the information listed in the Revdex.com letter submittedI found this complaint was filed by the daughter on her mother's behalf On 10/I contacted the member's daughter [redacted] to gather her mom's member information for me to begin my research of the matterAfter locating her mother [redacted] in our system I contacted the manager of our Case Management departmentIn my communication with the manager I informed her of the matter the member is having with getting a response from her assigned Case ManagerIt was explained the member's daughter is stating they have tried contacting two Case ManagersThe member states she has left messages for both of them and no one has returned her phone callsI requested the manager research this matter and look to resolve it for this member by contacting her to get the details as to how we can assist her On 10/I was informed by the manager of the Case Management department that she contacted the member's daughter, [redacted] ...and listened to her concernsAfter speaking with the manager the daughter stated that she is satisfied at this point and feels like the issues are resolved The manager provided her with her contact information so she can call her directly if she has any further concerns

1. Herb T***, I am not the salesperson, I was Tibbs Representative2.*** *** is correct in saying that the communication was poor and that on or about June 14th a proposal was written on his driveway to take care of a (1) sink hole and (1) edge Patch (x 18)equal to LF.
to apply PDC approxSY. That I did go out and meet with him and investigate the issue that he wanted to address. I said I would go back to the office to see what work was supposed to have been done. Not that that we had missed anything. I came back to office and wrote letter back to *** *** addressing the cracks in driveway, In error as I had not spoken with Dispatch on this work said that all issue would be addressed and taken care of the issues at same time. When job was sent to Dispatch work warranty work to be scheduled it was determined by Ken P***, the actual salesman that all work was performed per the contract/proposal. At that time it was determined that all work was done in accordance with proposal and manufactures specifications and that no work would be done. Also brought to companies attention that the home was up for sale. That's when the letter was sent to *** *** addressing the options for driveway work to be paid. I was transitioning into this position and I did quote the wrong total on discounted amount due. *** *** is correct that his disputed amount should be refunded to him. Tibbs will refund him $172.80. No other work will be preformed. Check will be in next check run next week. Please make sure that forwarding address is given for check to mailed to if home has sold and they have moved out. work is to be done

submitted. I found this complaint was filed by the daughter on her mother's behalf.
On 10/5 I contacted the member's daughter [redacted] to gather her mom's member information for me to begin my research of the matter. After locating her mother [redacted] in our system I contacted the manager of our Case Management department. In my communication with the manager I informed her of the matter the member is having with getting a response from her assigned Case Manager. It was explained the member's daughter is stating they have tried contacting two Case Managers. The member states she has left messages for both of them and no one has returned her phone calls. I requested the manager research this matter and look to resolve it for this member by contacting her to get the details as to how we can assist her.
On 10/8 I was informed by the manager of the Case Management department that she contacted the member's daughter, [redacted]...and listened to her concerns. After speaking with the manager the daughter stated that she is satisfied at this point and feels like the issues are resolved.
The manager provided her with her contact information so she can call her directly if she has any further concerns.

I am rejecting this response because: They sent a sales representative out to look at the driveway. He agreed there were areas where water would collect. He was even suppose to come when it was raining but he canceled that appointment stating that he didn't want to be out in the rain. He was trying to sell us the option to repair instead of being held accountable for not fixing the driveway they put in. I have never received a call back, no restitution. All I want is for those three areas to be cut and redone. We actually paid more than $800 for our additional driveway, which has nothing to do with this so not sure why that was made in the previous statement. They paved our driveway and part of it should have been repaired. As stated, when I started calling about 6 months after the driveway was done, I kept getting the run around and no one would call me back or pay attention to any of my messages. So here I am two years out of our warranty and still no one has addressed this. Had my original calls and messages been responded too, this would have been corrected within the warranty time. So I believe they purposefully ignored my calls and messages so that they would not have to come out and repair.

I have not received a return call from [redacted]. Due to the limited amount of information in the letter I do not have the ability to further research this complaint/issue. A member ID number, claim number or health plan name would provide me with the information needed to fully research the issue. At any time if [redacted] contacts me with the necessary information to research and provide next steps or resolution to this claim issue I will be happy to assist him.
