I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.
I am completely and utterly shocked that I am still being treated this way by a business. To be told there wasn't any truth to my complaint is asinine. There was not one piece of information that was not factual. I stated when and where and with whom these events took place. I can dissect what lies QHO Administration is telling just by these few statements. QHO stated," We do not dropped students for these issues on a one time basis..." when in fact, THEY DO! I had one write up and I was "terminated" from their program. I don't believe they gave me any time at all to correct I was doing "wrong".  Deb, one of the teachers, told me she was taking me to clinicals and was "excited to work with me" which are words that came out of her mouth while we were testing Heart rate and Respiration's. They kicked me out on a power trip. This decision was solely based on popularity. I got a along greatly with all the students in the class and the teachers seemed like they liked me, but apparently they couldn't stand me. Their only job was to teach me how to be a CNA. They failed greatly. I am a great student and I am going to be a Nurse. To be told by nurses that this profession is not for me makes me laugh out loud. I have worked in a CBRF for 6 years and I also work in the service industry, I know full well what I am capable of and their negative opinions of me were not warranted nor do they matter. The facts are I got kicked out on a one time write up and had $1080 stolen from me. To make matter worse, this is pushing my graduation date 6 months behind. I was planning a 2016 wedding and now even that is messed up because of QHO's poor ability to work with students. QHO is the worst business I have ever had to work with. I can't even name another business as bad as this one is. To boast how great the company is and how you have taught for over 28 years is remarkable, considering you couldn't teach me for 8 days. QHO administration claims they went "above and beyond" which is ridiculously false. I am extremely well liked by my bosses and have letters to prove it. (QHO teacher [redacted] told me I needed to ask people in my life if they liked me...???) As if my friends and family and coworkers would want to break me down like QHO did. For $1080 I was robbed, threatened, and bullied by one student who was removed from the program as well as by [redacted], [redacted], and the director [redacted]. I know a lot of people going into Nursing and I would NEVER recommend this company to any student. My mother works for a home care company and she is also not recommending QHO to anyone. From day one, I knew I was looked down upon because [redacted] looked at my engagement ring and said," that's a pretty big ring for a CNA." How could a teacher judge someone's life and profession from something so materialistic? Basically, she is saying "just a CNA" which is completely demeaning to those who are going to be "just a CNA". It is not professional to make personal comments about students. You would think she learned that in her 25 years of nursing. I recommend her getting some professional help with learning how to teach. I have had excellent teachers from Alverno, Milwaukee Area Tech, and MSOE. I know a good teacher when I meet one. The main problem lies with the owner. I have a video from when I entered their office both times asking for my money. The video shows that I did  not "scream" ONE SINGLE TIME. I was very calm and polite. To be told I was "creating an unsafe environment" by "screaming"  and is completely false as the video will show I am 5'1, 140 lbs small woman. [redacted], the owner, is at least twice my size. She probably loves the fact I got kicked out for one written warning because that's $1080 in her pocket. From what [redacted] said about [redacted] in class, we know she is an "oddball" and "eccentric". We were told she has millions of dollars and spends a lot of money on odd art. I'm so thrilled I just paid for her art. NOT!! I 100% believe she is fraudulent and is a bad business owner based on my experience. I want my full refund or I will be seeking advice from my attorney. Regards,[redacted]

Their name is Quality health care options Inc. Don't expect they are as good as their name indicates. They are too bad. They preach you CNA is a tough course. But CNA is an easy course as long as an instructor shows you each step how to handle a patient and how to take care of yourself. They are so biased. If one of the instructors see you while you make the first mistake, instead of telling you how to correct, she informs the rest of the instructors. So, the next silly mistake is enough for them to make you drop the course. We were 20 in number when we first begin the class. After 3 days, 4 students were forced to drop. At day six they told me to go home. You can guess the ratio how many students would graduate at day 16. Probably 4 to 6 students. They charge you $1,160.00 for 16 days class and force you to drop with in the first week. They are making unfair business.

Quality Healthcare option is the worst training school in wauwatosa wisconsin. They register 20 students for 16 days CNA courses. The first 2 to 3 days, they look friendly and after they make sure that every student has paid the whole fee, they start making students stressful in every lab practice. In class they focus in telling the bad part of working as a CNA .They focus teaching on negative side of patients and push students to stop training. If they know that most students pass written exam, they attentively try to find out a minor error in lab. Different teacher shows you different lab practice and this makes student get confused and make minor error with 6 minutes check out. As some students say and I observed too they can't manage 20 students with fewer teachers as the same time and they systematically encourage drop out. Imagine, 20 students are for 16 days and before the graduation of the first batch , the next other 20 will start class the following Monday and the cycle continues. They have fewer staff and limited class room and their system works more registration, more drop out and less graduate. Fewer students are easy to manage and during state exam, most or all will pass and they will be rated top in state passing rate. They are unfairly making money .So, you pay more than $1000 and end up with stress and waste of time and money . I remember a student forced to drop simply using cell phone in class in her break time. Another student was also forced to drop simply b/c she took picture in the compound. If they are sensitive for their rule, they would have posted their rule in clear area or tell you to put cell phone in bag or car. Trust me, they are happy when you make mistakes. I wish the gov't try to find out their unfair business and take action.

Review: I am a Nursing Student and I am required to take a CNA class to receive 120 of training as a Certified Nursing Assistant. I was told by my program director that I can take the CNA class through Milwaukee Area Technical college, or I could take it with Quality Healthcare options. After much planning, I decided Quality Healthcare options would fit my schedule, even though it costs more money than MATC does. The CNA class I chose is $1080, and is a 16 day - Advanced Paced program. After 8 days, I was "terminated" from the program for "attitude problems". I was told I would not receive a refund for "not following the programs guidelines". However, I believe the teachers and the Director of QHO did not follow their guidelines either. On their "violation records sheet" it states," 5 Violations (Including the "written warning")= Class Termination. I only received one written warning on 4?27?2016 "for not taking feedback gracefully". This was the 6th day of the 16 day class and when I wanted a copy of my "written warning" I was told I was not allowed to have a copy because it did not belong to me by [redacted], one of the instructors. Another student, [redacted], tried to take a picture of him written warning and was withheld from lunch to be scolded for asking for such document. I believe their teaching creates a hostile environment as students are not allowed to ask questions or ask teachers to re-demonstrate skills. This leaves students confused and nervous since skills are required to be executed flawlessly for the State test. I was terminated by the program director who called me on a Saturday multiple times before I answered my phone. She didn't ask me any questions about the program (I was very unhappy with how the teachers "verbally warned me on 4/27) One teacher, [redacted] told me that "nobody wanted to work with me, and nobody liked me" because I was "arrogant" and a "know it all". She also explained to me that [redacted], the program director, stated that she would not hire me for her facility. I asked her what I did wrong that makes her think I am arrogant and she said it was my attitude. Sadly, I believe she has a very poor idea of who I am as a person. However, I did not pay $1080 for her opinion. I paid QHO for an education that I did not receive. On top of not receiving the training I need to be a Nurse, QHO stole my money and will not return it. [redacted]s idea of "constructive criticism" left me crying out of anger and embarrassment because I have never been talked to like this before from an authoritative figure. The following day (4/28) a teacher named Deb pulled me aside and explained to me that she read my "written warning" and she wanted me to know that she does like me, and she was excited to "take me to clinicals" with her. This comment made me feel much better as I was very scared to come to class because I felt as if I was being bullied by 3 teachers ([redacted] the director, [redacted] a teacher, and [redacted] a teacher). Following the 4/30 phone conversation I had with [redacted] the director, I went to QHO to receive a refund of my money and was denied again my "written warning" as well as my $1080. On the receipt for the program states a section of incidences that students will not receive refunds if and one of the incidences includes students being terminated from class for violations. However, I did not receive the number of violations required to be dropped from the class. I believe this company is fraudulent and kicks students out for minor violations and takes their money. I believe every student in the class believed the same thing. I was told to leave the premise when I asked WHO I could go to complain about this company. I was ignored and eventually threatened by [redacted], the owner who called the Wauwatosa Police on me. I was informed I would get what I needed in the mail. [redacted] made the situation hostile by coming around the desk and standing directly next to me in a threatening manner. I told her I believe she is stealing peoples money by "terminating" students" for very minor violations, and including not giving students the number of chances written on the Violations sheet. I was on time to every class. I participated well, got a long with other students, and took two tests where I scored a 97% on each test. I believe I would have been successful in this class had I been given the chance to stay in the class. I believe they conducted very poor business with me, as well as other students, and the matter should be investigated. I am just one student who paid over $1000 for a class and did not receive the services I paid for. How many other students have dealt with the same issue?Desired Settlement: I want a 100% refund of my money for this class. And an apology would be great considering this company set me back 6 months of school which is delaying my graduation which is costing me thousands of dollars.

Response to

complaint:

QHO administration read through the

complaint written by this past student.

Unfortunately, so many of the details and listed events for the

complaint are false. She was dropped for

her divisive, disruptive, defensive behavior towards the instructors and the

program director. We, like any other

program, have expectation of the students.

These include being respectful, cooperative and to be able to take

feedback without becoming defensive.

Qualities like these are imperative to this profession. Throughout the program, we do several group

units on this topic and we work with students on an individual basis to help

them achieve their best chance at success.

These expectations are given to the students prior to class and on the

first day. They sign off on these

expectation prior to class and on the first day. It is clearly noted that if a student is

dropped from the program for a behavioral issue they will not receive a

refund. It is also clearly noted that

there is no “number” of times a student is allowed for behavior issues. It is based on the severity of the issue. This is covered on the receipt and the

“expectation” contract that is signed by the students. However, in her case, the instructors gave her

multiple chances to improve her demeanor and succeed in this class. The “violation” form that is referenced here

is used to document corrective action plans and other rule issues such as

tardiness, uniforms, phones, etc. We

don’t dropped students for these issues on a onetime event. After a certain point, the student would be

dropped if the issues continued. This

process is explained in depth as well. She

has a copy of her contracts, rules, write-ups, and the receipt regarding the

refund policy that she signed.

This student had issues with all the

instructors. No instructor stated they

felt comfortable continuing with her at clinical. When the program director called her to

discuss what was happening in class, she continued to distort the truth and

would not consider changing her behavior.

She did ask what she “should do” if someone pointed out that she seemed

defensive. She stated she should be

“offended” and had the “right” to be “defensive”. The director tried again to give some

constructive advice and stated: “I think I would wonder how I could change that

perception and ask for some feedback.”

(This conversation was witnessed).

At that point this student swore at the program director.

On the day she came to QHO, she

demanded her money back. She was very

hostile, loud and demanding her money back.

The director gave her the phone number for filing a complaint and then

asked her to leave because there was nothing else they could do for her at that

point but she refused and became really disruptive to the entire office. We asked several times and again, referred to

the proper complaint procedure. She

refused and was creating an environment that was unsafe. The owner did come down and call the police. At no time did she threaten this person. She only again stated “you need to leave,

this is private property”. She continued

to scream at the owner that we were frauds, stole her money and she wasn’t

going to leave. Eventually she left when

she realized the police were coming. The

director did mail her all the information for filing a complaint.

It is difficult to respond to all the

false allegations made by this past student other than to say, these were made

in an attempt to distort actual events, and minimize her responsibility in this

difficult decision that had to be made. The

faculty went above and beyond expectations to assist this candidate. She was given an extensive amount of time to

take ownership of her behavior. Thankfully,

all of the interaction with this student were witnessed and have been

documented.

As to her desired settlement, because

she defaulted on her responsibilities and contract, she would not receive a

refund. Tuition includes the staffing,

books and supplies, which she received.

She did not pass the class due to her own conduct.

We do not take these decisions

lightly, nor are these actions required frequently. Due to the short duration of the course, it

is uncommon for issues like this to arise.

We have been training candidates for 28 years. We have an exceptional reputation in the

community because of the quality candidates that come from our program.

Sincerely,

QHO Administration.

Response:

Review: I signed up for a pre-paid class through the FSET program provided by the State of Wisconsin. The plan was to go to school for three whole weeks, take the test and pass it, then go on to obtaining my CNA

re-registered licensed that I needed to up-date since 2007.

However this place has been failing good law abiding, smart students, who have all passed the written test, and now are only failed if they don't stand an inch or so according to their own testing rules. I

know that student's can be penalized, but to be totally thrown out of a program for just under 3 or 4 mistakes other than that of a written test is totally outrageous.

Help, I want my Re-Certification license - that I may work again.Desired Settlement: I deserve to be enrolled still in the program, with the chance of pass or fail, on the written part, for which one should be held according to. Not flunce every body in the first week of a 3 week session.

So I am asking for a second chance. A chance to do every thing that they said I should do. Stand properly, lift down the bed side rail, and thirdly to physically turn the bed down - when I was not

told to physically do that in the first 4 day's of trained teaching - all in just 1 week. the 5th of August till now 9th of August. I still have 2 more weeks to go. $1300.00 have already been paid for me,

and I want my chance, since I have been given this opportunity.

Response:

Review: After one day in this 16 day Certified Nursing Program, I was blatantly disgraced by the instructors and told that I had not done the first skill perfectly after 3 tries and I was not going to get more than 3 tries in the program. This was on the morning of the second day. After, this disgrace I did not return for further abuse, embarrassment and discrimination. My course funds were not returned but taken by this business in full. I sacrificed time and my own resources in testing, taking drug and TB tests and giving up 2 jobs I had previously held in order to have the day hours free for the 3 wk course. I consider this business to be totally unprofessional and not worth even considering by any future applicants for the CNA program.Desired Settlement: To the WOW program at WCTC in Work Development a refund of the entire cost of the training and supplies.

Thank You

I recently took the full time Certified Nursing Assistant training and had a very positive experience. I liked that the full time training class was only 3 weeks long, including clinicals, Monday through Friday. This class is ideal for anyone also enrolled at a college with traditional fall and spring semesters and is looking to complete CNA training on their summer break. During the program my class had several different instructors that all imparted different knowledge from their experiences working in hospitals and long term care facilities in addition to teaching the curriculum in a clear and concise manner. At QHO they give you ample time to practice the skills you will need to be successful at clinicals and to pass the state licensing exam. They also teach things that aren't on the licensing exam but that you'll need to know how to do once working in a LTC facility or hospital. The instructors I had were friendly and professional. The approach they took in clinicals was one of increasing the student's level of responsibilities day by day so we were able to learn and do a lot while not being thrown into a situation without solid skills and direction in place. On the last day, 100% of my class graduated. I then passed my licensing exam and feel the instructors prepared me well for the exam and a career as a CNA. Lastly, I noticed QHO offers part time night and/or weekend CNA training classes as well. While they are longer than 3 weeks, they offer shorter class days and a degree of flexibility for students with other work/school commitments during the day.

I have never felt so discouraged while taking this course. When asked to re-do something several times, the "instructors" should NEVER have an attitude, let out big sighs and treat the students like we're dirt. It's UNPROFESSIONAL. I don't even believe half of those instructors are professional; and certainly don't act like it. I could easily teach myself from the book and get by better than them "teaching". This was one of the worst possible places for taking this course, and it certainly was a total waste of my money. For the ridiculous amount of money, they should provide caring, happy, and pleasant nurses/instructors. For them to sit around wasting MY time talking about what they're doing after work is ridiculous. I would definitely not recommend QHO to ANYONE.

Review: I was enrolled in the CNA course this summer. On the second morning of class first thing I was asked to repeat a skill we had learned the afternoon of the first day when I had practiced it twice. There was zero tolerance. It had to be perfect and I was told by the instructor that this had been a test and after 3 imperfect tries, I would be failed from the course. A little later I was asked to sign a WRITTEN WARNING for wearing nail polish even though no one had given me an oral

warning. The 3 X and your out requirement was not written in the so called "yellow book" of instructions given to me the day before on the first day of class. The nail polish rule was there but these strict requirements were

not announced or read on day one to the class and the homework given to the class for the first night did not include this chapter of the yellow book. Another student learning another skill was allowed to do it many times over-

4 times for one skill that I witnessed and told by the instructor that the instructor would stay with her to help her learn it if it took all afternoon of the 2nd day. I was so aghast, embarrassed and blind sited that I could not go back the 2nd day.

This was blatant discrimination against me. The school is only interested in their positive performance of their students in being prepared and passing the state CNA test . Also the company's bottom line is all that matters. The school called the agency where I was tested and refered and told them I had not shown up the 2nd day for class and they would keep my tuition fee and supply fee which amounted to $1200.00Desired Settlement: Full refund to WOW- Workforce Development Inc to the attention of Angela Brill, 892 Main St, Pewaukee, WI 53072 in the amount of $940.00 which is the amount they paid ( They also paid a $260 supply fee which s/b

refunded as soon as I return all supplies to the center).

Response:

My name is [redacted] and I am the Education Director at Quality Healthcare Options in Wauwatosa. I am responding to complaint #[redacted] from [redacted]. In addition to this response you will find our course rules, guidelines and expectations

for passing the course. Anyone who registers for the class is given the rules prior to the class. I also attached the day one instructions that are covered by the instructors relating to the expectations for passing the class and skills competency. On the first

day of the class we also go over the same rules they received at registration. You will find these forms and [redacted] signed agreement that she understood and agreed to those terms. The C.N.A. training course is regulated by the Wisconsin Department of Health services. All training sites require students to pass written exams AND competency on skills as well as clinical practicum. [redacted] has a contract with Quality Healthcare Options in regards to the financial policies we hold. We only bill for

students who attend the first day of class and we have a non-refund policy except for the test fees. For this reason, we assure that the students who are interested in the course are aware of the expectations. The course [redacted] enrolled in was a 16 day

program. This decision was made by the student and her case worker. I will address her concerns starting with the competency of skills. On day one, each student is given a packet with all the assigned skills. This packet goes over the steps of each skill in detail and we review those with the students. We also explain the importance of infection control, safety and dignity issues as it relates to our ability to care for real clients. We explain (see day one instructions) that on each day, the instructor will demonstrate the assigned skill. Each student is given at least 2 attempts to demonstrate comprehension after the demonstration. If a student is unable to pass on that day we review the skill with them. They have the evening to review their skills sheet. The need day is they have a second attempt to demonstrate comprehension. When a student is struggling, the instructor makes time to share observations, concerns and suggestions with the student in private. If the student is unable to pass the second attempt, the skill is again reviewed and the student comes in the third day for a final attempt. Instructors make time before class starts for lab and competency testing. We have approximately 2-4 skills per day to complete. We start with basic simple skills and build on those principals as we go through the program. The standards for passing competency are set by the Department of Health Services.

In [redacted] case, she was unable to demonstrate principals of hand washing and putting on and laking off gloves and gowns on the assigned day. She was given 2 attempts and was directed to come in the next day to attempt these skills again. We

also spent extra time with her on documentation of vital signs and filling out the worksheets. This was done in private. [redacted] was advised to review those skills in the skills packet.

On Day 2 (which she did attend contrary to her complaint) she was again unsuccessful in retaining and demonstrated these skills. The skill for the second day is making a bed. [redacted] was unable to complete that skill after 2 attempts that day. The instructors

had concerns because this student was struggling with concepts of safety and infection control. The instructors did talk with [redacted] during break about these concerns. We discussed the plan for her to have another attempt at hand washing, gloves and gowns on the 3rd day of class. Also, she would have a 2nd attempt for bed making that day. [redacted] did express that she was set in her ways and that we shouldn't be too picky. We explained the need for guidelines in healthcare and principals for the safety

and well being of our clients. We again, reviewed the skills handouts with her.

As to her complaint regarding the nail polish. Please refer to the policy attachments and her agreement. The student was aware of our policy prior to the class and again on the first day of class. It was the second day of class that she received a warning for nail

polish. The instructor talked with her privately and [redacted] again, signed that form. She did verbalize to me that she didn't agree with this policy. We have this policy for infection control issues. As with any school or company, students and/or

employees are not able to determine which rules or protocols they will or will not follow. On the 3rd day (7/25/13), [redacted] did not attend the class. She stated this was because she was aghast, embarrassed and blindsided. I can assure you that any

counseling we did with [redacted] was done in private. She clearly was aware of the expectations for this class. I understand that it is disappointing and frustrating when we are unable to cor1'plete/pass a program or course. There are necessary requirements

set forth by the Health department to ensure that Certified Nursing Assistance have demonstrated competencies in theory and skills. Our program is an approved C.N.A. program and must adhere to those standards. On occasion, we have a candidate that

cannot complete the competencies necessary to complete the course and they are dropped from the program. In [redacted] case, she dropped herself from the program. The tuition agreement is with Workforce Development and resolved through the contract we have with them. They were contacted on the day [redacted] did not show up for class. They did have a meeting set up with this student, but the student did not show.

If there are any other documents you need or you have any other concerns, please don't hesitate to contact me.

Sincerely,

Director of Education
