Due to a data processing error, a 1099SA form was created and mailed to Mr*** *** for the tax year of 2016in the amount of $6,Upon investigating this issue, we realized the account had been closed, therefore, a 1099SA should not have been issuedSince, we issued a corrected form
to Mr*** with a zero amount of distributionAvidia Bank has taken the necessary steps to assure this issue will not happen in the futurewe apologize for the inconvenience this may caused Mr***

The wiring was not originally done to the specs I provided Rudy (attached) or I wouldnt have had to reach out to you The documentation stated ALL wires needed to be size Kyle, subcontractor - click electric) is coming to fix tomorrow 5/23/After that, I will have to drain the spa, refill it and test the spa to insure it works and does not overheatThe spa guy stated that if it overheated, it should have tripped the breaker and shut it down (for safety purposes)This did not happen at the emergency shutoff OR the main breaker; therefore, the issue still could have been related to the electrical wiring not being the correct sizeI will let you know if the test is successfulIf it is, we can close this caseIf it's not, I will ask that Kyle/Rudy, the spa tech and myself meet to look further into the issueI don't want a finger pointing situation Thank you!

Dear Sir or Madam,Ms*** *** is a valued client of our company and we have taken the appropriate steps to alleviate her concern over the original wiring of her spa Immediately upon her contacting our company, we attempted to make arrangements to have the electrical contractor visit her
home to review the original work GPS has photographs of the installation performed in May, 2016, and those photos verify the use of the proper gauge of wire as per the manufacturer's specifications Still, Ms*** has experienced a malfunction of the spa and it was appropriate to inspect the wiring connections to determine if they might be the cause or not Today at 3:pm, the principal of Click Electric, Kyle H*** and GPS' Senior Project Manager, Rudy A***, visited with Ms*** at her home for the sole purpose of making a determination regarding the wiring installation for the spa It was determined that the wiring was installed properly and that all wires that carry current are, in fact, gauge product There is no corrective action required and the wiring was removed from consideration as a cause for the spa's current malfunction MrH*** did offer to exchange the gauge ground wire for a gauge replacement, this will not affect the functionality or performance for the grounding system, but was offered to ensure that there are no wiring issues - of any kind This work will be performed on Tuesday, May 23rd, as agreed to by Ms***.We have written Ms*** directly to confirm today's findings and to ask if our efforts have satisfied her claim We await her response.Sincerely, Michael H***Managing DirectorGPS, Inc
