December 30, 2015Dear *** ***: I have completed the investigation for the two accounts pertaining to *** ***’ complaintFor the initial visit, (*** ***) she presented with a different complaint than that of her second visit (*** ***)It is for this reason that her diagnoses
were differentA claim was sent to her insurance carrier for processing for each visitHer insurance made a payment for her initial visit (*** ***) but denied the second visit as inclusive. *** *** contacted our office on May 13, and requested that a modifier being added to her second visit so that her insurance could reprocess the claimOn the same date modifier ** was added to her visit and a corrected claim was submitted to her insurance on May 16, On May 28, our office received a denial from her insurance stating that this second visit was inclusive to her initial visit. On September 30, *** *** contacted our office and inquired why her insurance only paid for one visit; it was at this time our office sent her insurance carrier an appealOn October 21, we received notice that our appeal was denied*** *** contacted our office on January 19, and was advised of the insurance denial; we suggested that she file a grievance with her insurance carrierShe also stated that she would send a dispute to our office. We received *** ***’s email disputing her billShe requested that the account be removed from collections and that this was a fraudulent chargeOur office removed her account from collections as a courtesy and *** ***’s account was reviewedAs stated within her complaint she was treated twice on the same date of service which incurs a bill for each visit.
