April 11, Dear [redacted] , Thank you for the opportunity to respond to the complaint about billing/collection issuesOur records show that this patient was seen six times in and received up to statements from March of to April of 2015, each of which showed a patient balance, starting with $on March 18, At no time did this patient receive a bill with a zero balanceIn fact, our billing software would not even send a hill to a patient with a zero balanceOur records also indicate that the last payment we received from the patient was a check for $on Jan6, 2015, at which point there was still a balance of $Our call logs show that in May of 2015, our employee who was dedicated exclusively to contacting patients with overdue balances reached out twice to this patient - on May and May - explaining that if no payment was made we would turn over the account to the collections agency, which we did on January 26, I spoke with the person in charge of this claim at our collection agency and was told that each of the four visits that have remaining balances has its own line on a credit report a common practice in the industryFinally, must say that we have always prided ourselves on our relationship with our patientsIn our years in business, we have never had this kind of complaint lodged against usUnfortunately, neither the billing manager nor the office manager with whom this patient dealt are still with us, and we can find no record of calls and/or letters on this matterWe apologize for this, as well as For the inadvertent error that resulted in this patient receiving another person's billWe are a small company, and we do all our billing by handOur patients' information is sacred, and we deeply regret violating this trust, even though it was completely unintential and no private health information was revealedThank you in advance for your kind attention to this matterSincerely, Jennifer I [redacted] , Owner Rebound Physical Therapy

balance, starting with $81.98 on March 18, 2014. At no time did this patient receive a bill with a zero balance. In fact, our billing software would not even send a hill to a patient with a zero balance. Our records also indicate that the last payment we received from the patient was a check for $37.00 on Jan. 6, 2015, at which point there was still a balance of $70.95. Our call logs show that in May of 2015, our employee who was dedicated exclusively to contacting patients with overdue balances reached out twice to this patient - on May 20 and May 26 - explaining that if no payment was made we would turn over the account to the collections agency, which we did on January 26, 2016. I spoke with the person in charge of this claim at our collection agency and was told that each of the four visits that have remaining balances has its own line on a credit report a common practice in the industry. Finally, must say that we have always prided ourselves on our relationship with our patients. In our 11 years in business, we have never had this kind of complaint lodged against us. Unfortunately, neither the billing manager nor the office manager with whom this patient dealt are still with us, and we can find no record of calls and/or letters on this matter.
