patientsThis requires both parties to have an open, honest involvementYou were advised of *** *** *** decision to transition services to DrRandy R*** as of April 1, (enclosed)To reinforce the commitment to a seamless transition, RFP honored the membership premiums and co-payments without any changes to your feesIn addition, you received added benefits by now seeing by a physician instead of a Family Nurse Practitioner (FNP)an increased value for the same membership fees with access times per week vsthe former days per week previously available to youYou began the relationship by initially requesting a medication refill (on 5/1/2015) which was filled with the caveat that you set an initial appointment with the doctor so he may get to meet and examine you to confirm you were receiving the correct medication for your particular situationThe appointment you made was for May 26, at 2:00pmYou did not show or call to cancel or reschedule your appointmentIn reviewing your chart, this was typical for required appointments with *** *** ***A letter was sent to you from RFP advising our "Cancellation and Appointment" policy on the evening of the 26th of May (enclosed)After not showing for your appointmentyou called the next month and asked about having a Depo-Provera injectionYou were advised we did not have that medication at our office and were referred to *** *** as they offered this service at no charge (as *** *** *** had referred you, and you attended on many occasions)You were never told we "didn't give shots" as we provide many injections throughout theweek at our practiceWe just do not carry this medication due to low volume of need for such an injectionThe contract you had with *** *** *** was carried over when RFP assumed the businessand it's patientsYou did not have an issue with this when the premium was pulled in May or June of as we did not hear from youIt appears the that this issue began when you 1) received your cancellation policy and 2) you did not get an injection you wantedYour claim that our staff would "hang up" when you said your name is difficult to believe as our staff respects all who call our clinic and do not disrespect anyone, regardless of the nature of the callAs for the charges, if we did charge your card for the month of July, I apologize and will refund you the $(enclosed) you claim to be chargedAgain, this was never brought to our attention so we could not address your issue, and we do not charge members that have terminated their membershipsWe wish you the best in health and in your future, and thank you for allowing us to serve your health needs
