September 16, 2014In regards to the complaint filed by [redacted] , in regards to Reflection Dental — [redacted] ; we take our patient's concerns and complaints very seriouslyWe would like the opportunity to respond to the patient concern in an effort to resolve and ensure [redacted] is aware of the balance her account currently holds.As a courtesy to our patients, our team members provide each patient with a copy of our financial policy as well as present detailed treatment plans showing expected co-pâysThis information was provided to [redacted] at her appointmént with us on January 6, with Guardian In-Network FeesAs previously discussed we have no record within our office of [redacted] 's insurance with [redacted] DentalOnce we were made aware of the insurance change, we did contact [redacted] Dental for a full break down of coverageH was at that time, March 8, we discovered [redacted] had a waiting period of year with Deltä Dentaf.When filing a claim on [redacted] ’s behalf, the proper procedure codes were submitted regarding her extractions [redacted] had both teeth numbers and extracted by the description of erupt teeth and teeth numbers and surgically extracted by the description of impacted teethUnfortunately, due to the waiting period with [redacted] Dental [redacted] has a balance within our office of $1000.00.We strive to serve our patients and provide the highest quality of patient careOur office sinceresy apologizes for this situation and hope this response further explains the current balance for [redacted] .Sincerely,TaShawna SClient Administrator

[redacted] had both teeth numbers 17 and 22 extracted by the description of erupt teeth and teeth numbers 1 and 16 surgically extracted by the description of impacted teeth. Unfortunately, due to the waiting period with [redacted] Dental [redacted] has a balance within our office of $1000.00.We strive to serve our patients and provide the highest quality of patient care.
