August 13, 2015Hello,In reference to your written notification dated August 10, (received on August 12, 2015), you stated that you have attempted to contact me about complainant [redacted] by mail with a letter dated 7/30/and via email (no date referenced)Unfortunately, neither communication attempts reached me.This issue of communication attempts from the Revdex.com has been a persistent challenge because in all cases, I've either never received the initial notification alleged from your office or you have never received my initial response to a complaintI have appealed to your office numerous times about how to contact meI have also appealed directly to the Council of Revdex.com (CRevdex.com) for added guidanceAfter my appeal to the CRevdex.com, communication improved but seems to be an issue once more.Again, I can be reached at [redacted] [email protected] at any time should you need assistance with a complaint about PTCBPlease update your records.As for the complainant, he was considered a "no-show" on his testing date of July 29, According to PTCB's no-show policy, "candidates who fail to appear for their scheduled exam appointment will forfeit all feesCanceling and rescheduling is handled directly with [redacted] ***." However, candidates may request an emergency withdraw of their scheduled exam appointment provided they submit a request to PTCB no later than hours after the last day of the candidate's authorization periodOnce approved, candidates will receive a refund minus a $processing feeExamples of what PTCB considers an emergency include serious illness or injury, unexpected hospitalization, death of an immediate family member, a serious accident, or court appearanceAll of PTCB's policies and procedures concerning the exam may be found at www.ptcb.orgThus, [redacted] 's request for an exam registration refund is denied.Thank you,Eleanor DManager of Customer Service

July 23, 2015Hello,In reference to your written notification dated July 10, 2015, Revdex.com complainant [redacted] has requested to "take my test ASAP, or have the order cancelled ASAP, and my money returned ASAP." [redacted] 's initial complaint about her exam application was received via telephone call to PTCB's call center on July [redacted] 's issue was escalated to our certification manager for better assistanceAccording to the certification manager [redacted] 's payment was applied and the exam application approved on July 12, And as of July 12, 2015, [redacted] was able to schedule an exam appointmentThus, any consideration of a refund is denied.I can be reached by email at [redacted] @.ptcb.org should you have any further questions about this case.Thank you,Eleanor DManager of Customer Service

RE: ID#***Hello,In reference to your emailed notification dated June 5, 2017, complainant *** *** ***, a Pharmacy Technician Certification Board (PTCB) Certified Pharmacy Technician or CPhT, submitted a renewal of certification application on July 28, *** ***’s
certification expired on July 31, All recertificants must renew every two years in order to maintain an active PTCB certification.There are certain requirements regarding PTCB’s Renewal of Certification policy, including the submission of reporting only twenty (20) pharmacy technician-specific (T-specific) continuing education (CE) by their respective due date in order to receive a successful renewal of certificationThe T-specific requirement was implemented on January 1, *** *** submitted an application that included CE’s which are not T-specificThese CE’s are dated as being earned from April to June Although PTCB allowed CPhT’s to report CE’s that are earned prior to implementation in 2015, the majority of *** ***’s CE’s reported that year were dated after implantation, which prompted the application’s return for correction by PTCB’s certification teamSubsequently, PTCB forwarded an email on August 8, 2015, regarding the status of the application and instructions on how to correct the issues that were foundAlso in the email, it was stated that *** *** had days from the date of the email’s initial release to correct and resubmit his renewal application for reviewUnfortunately, PTCB never received a replyAs a result, *** ***’s certification expiredHe had until July 31, 2016, to reinstate his status, which also lapsedCPhT’s who lapse in reinstating their certification must retest in order to become an active PTCB CPhT againBecause of these factors, a refund of *** ***’s renewal payment from is deniedNote CPhT’s who are successfully certified by PTCB must maintain the renewal of their certification and adhere to all requirements accordinglyCPhT’s are also responsible for maintaining their own current contact information, including mail and email addresses, on their PTCB accounts.Thank you,Eleanor JD***Manager of Customer ServicePTCB
