First and foremost, I believe Ms***'s accusations and statements are mostly and misleading
This is my response;
Before I see any new patient or initiate any dental procedure, I have them read and sign a financial agreement with office policiesThis acts like a contract between the patient and myselfI must also follow State Board Rules and Regulations by making sure all potential patients sign a treatment plan consent formThese items are completed before I put a mouth mirror into their mouths(see items and 2)
Ms*** was seen on 8/13/for a new patient exam and xraysHer diagnosis did not require a code procedure Therefore, a code was not intiated nor filed with her insurance company as this would have been insurance fraudI billed her insurance for codes and 0210, which I did performI also sent a pre-authorization, at Ms***'s request for the recommended procedure, so that she would know in advance what her insurance company might pay( see items # and 4)
On 9/9/2013, the Explanation of Benefits paid me and stated that Ms*** would owe me Ms***'s first statement was mailed to her on 9/9/( see items #and #5)
Finally, months later, after many phone calls and statements sent to Ms***, she mailed me a bounced check for (see item #and 7)
After the bank charge, the balance and the billing charges, this left her account balance at
My front desk person, by mistake, made the first billing charge at instead of I caught her mistake on 2/20/and credited her account by ( see item #9)However, please notice how I tried to wait as long as possible before these late charges were assessedI had given her months instead of days
On March 3,she promised to send me a money order for no later than Wednesday April 2"( I even made myself a note in her account to give her an extra day.) On Friday, April 4th it never came so I gave up on Ms*** and felt her too untrustworthyI therefore decided to cash a hold check she had given me for thinking that it too would probably bounce but took my chances anywayI did not place this payment into her account because I truly did not think it would pass my bankI was shocked to see by the following Tuesday, it cleared
The money order of was promised to me by April 2ndIt finally reached my mailbox days later
Ms*** was charged the for her returned check per our original financial agreement( see item #1)
Ms***'s account was also credited for on 4/14/After all other necessary charges such as costs of certified mail and stamps, and time consuming phone calls made by my front desk person, she was left with a credit of I promised Ms*** via a phone call that she would receive any refund due her by the end of the month, as this is when I close End of Days and End of MonthsThis is what I followed through with, an agreement, and her check of was mailed to her the afternoon of April 30th via certified mail(see item #8, item #again, and all miscitems enclosed)
