Tell us why here The services in question come from 10/28/and 12/23/for [redacted] The majority of the insurance funds were applied towards the patient's deductibleStatements were mailed to the patient on the following dates: 12/4/15, 1/1/16, 2/26/16, 3/25/2016, and 4/22/No response was rec'd from the patient to those statemetnts and on 5/27/the account was assigned for collectionsThe patient's mother first call to the clinic came on 9/9/and since the account had been with collections for almost months we required the patient to deal direclty with the collection agency.Our statements are coded that if they cannot be delievered to the patient they must be returned to usNone came back and we have confirmed that the mailing address they were sent to is correct

The patient was seen in the offices of DrH [redacted] on 12/13/Currently the patient is not being billed for these services as the insruance company is still processing the claimWe also have no recorded note that the pateint has called and talked with our officeThe visit was not as simple as discribe by the patientAfter a detailed examination by the Doctor an endoscopy of the left nasal cavity was performed to determine if indeed the patient needed surguryif you have an furthur questions please feel free to contact DrH [redacted] 's office

I am currently out of the office until April 10lth, however, there are severl individuals who are more than willing to assist you with this matterI am wondering if it might be better to disuss this over the phonePlease call 801-375-and ask for Patient ServiceIf you would rather to continue to correspond through this service I will get back with you when I am back in the office

The services in question come from 10/28/2015 and 12/23/2015 for [redacted]. The majority of the insurance funds were applied towards the patient's deductible. Statements were mailed to the patient  on the following dates: 12/4/15, 1/1/16, 2/26/16, 3/25/2016, and...

4/22/16. No response was rec'd from the patient to those statemetnts and on 5/27/2016 the account was assigned for collections. The patient's mother first call to the clinic came on 9/9/2016 and since the account had been with collections for almost 4 months we required the patient to deal direclty with the collection agency.Our statements are coded that if they cannot be delievered to the patient they must be returned to us. None came back and we have confirmed that the mailing address they were sent to is correct.

I will make contact via phone as time allows or wait until you are back in the office. I am not marking this complaint as resolved until it actually is.
