Daughter was told charges $712 per crown. She paid one of them at this price and office billed insurance at $1300 per crown.
This is the email I sent to the doctor that summarizes what happened.

I am very shocked and disappointed the way your office has billed my daughter's two crowns. I have called several times to discuss this with you and have never received a response except you are busy as the receptionist relayed to me. I have discussed this with [redacted] many times to get this cleared up and have not had any resolution. He has told me things that were not true such as the insurance company sets the price and now I found out he had called my insurance company to tell them he told my daughter the inflated price which she never did talk with him on that day. He told me he called my insurance company, and they explained the benefits. My insurance company stated he never discussed changing the price.

We have a copy of the cost that was quoted to my daughter of $712.00 per crown and a copy of the receipt she paid for one crown on September 26 based on the agreed price. The balance due was showing zero with the estimate insurance at the bottom of $824.00. Ashley received this from the receptionist. Ashley agreed to do both teeth based on the price given to her of $712.00 per tooth .

On November 14, you sent out a new bill with totally new prices that were almost double the agreed price per tooth. Also, this new price of $1300 per tooth was reported to our insurance. This is an act of fraud charging Ashley one price and then billing our insurance with an inflated price that was not the agreement. Not only that, but you also billed the same tooth twice to my insurance. I have been trying to get this resolved with you all for several weeks and am not getting anywhere. You all have set a deadline to pay the inflated/incorrect amount by December 14. I have told your receptionist, [redacted], that we want to pay the corrected amount. After discussing this issue with my insurance, they said that they need for you to send them the corrected amount of $1424.00. They also said they could pay more on Ashley's account since it would not be maxed out. We may not even owe you anything at that point or receive a refunded amount.

Since I cannot seem to get this resolved, I am reporting this to the fraud line and other agencies to get this corrected. I cannot believe it has come to this and have never experienced anything like this with anyone. This is a very simple process and can be corrected easily but you have made this very difficult.
