I love going to Dr. KATNIK's office. When I have pain, he goes to great measures to alleviate that pain ASAP. I have been going to him for over 30 years. Dr. KATNIK is professional and gentle when having to numb me for treatment. The office makes sure that everything is perfect before I leave, because I travel over 70 miles to see him.

Ms. [redacted]'s complaint with our office is regarding her
daughter, [redacted]. On Tuesday February 5th,2013, we
saw [redacted] in our office. Upon signing in, the patient wrote that
her insurance had not changed and Safeguard Healthy Families
had the patient listed as being eligible to be seen In our office. We
submitted to Safeguard Healthy Families for the patient's visit,
however,the date of service was never paid. Our office even
faxed a copy of the patient's claim to her Insurance, again, to
which the insurance responded with a reply of "patient was not
eligible for benefits on the date of service indicated". Enclosed,
you will find a copy of the patient's sign In,her claim,and even a
copy of her chart detailing our attempts to bill the patient's
insurance. If you have any questions or concerns regarding this
issue, please contact our office.

Review: I went to Dr. [redacted]'s office because I was having problems with a food trap. He recommended a bridge be put in. However, when I went to the appointment they said it was a crown.When the crown was put in, it was very uncomfortable and didnt sit right. It started to affect my bite and was biting the inside of my mouth. He agreed to try to fix the problem and attempted to try to do so 3 times.After the third time and a lot of pain I didnt feel that I could go back to him as his hands were constantly shaking and he couldnt position the crown correctly. He also did not take xrays to make sure the crown sat correctly.When I went to another dentist after dealing with the pain, I was told that the crown had not been positioned correctly and that there was food getting trapped under the crown causing an infection and that the crown would need to be replaced along with a new crown for the tooth in front of it. When the new dentist started the procedure I was in so much pain they had to refer me to a specialist-an endodontist who advised me that I needed a root canal. Two root canals later, (the tooth next to the original crown Dr. [redacted] had put in also got infected)I've now paid $1300. $435 for each crown and $215 for each root canal. Based on the information my new dentist gave me, I would not have had this issue had Dr. [redacted] properly placed the original crown.Desired Settlement: I would like to request a refund for the amount that my insurance paid Dr. [redacted] for the procedure that he incorrectly did, causing me further expenses. Documentation from his office would show that he attempted to fix the problem three times --unsuccessfully. I also called and told the office that I would no longer be their patient due to this issue.
