I'm writing in response to the complaint that was submitted on May 2, and was assigned an ID #of [redacted] The patient had an initial consultation and exam on March 11, At that time benefits are verified with their insurance companyMom was given different payment options one for full treatment regular braces for months and upper treatment only for monthsWe participate with her insurance so we gave her the participating rateThree weeks later Mom loses her insurance, and states she will obtain cobra InsuranceI recheck on her insurance again on Thursday April 28, and it was still terminatedI sent out a new payment option to Mom with our fees and not including insurance benefitsOn May 2, the patient and Mom had a pre-treatment diagnosis with DrThe bill was different from the original payment options because of no insurance coverageI explained to Mom because she didn't have insurance we could not give her the PPO FeesWe were asked for a refund on the money Mom paid for records and explained that a service was provided and that a refund was not going to be given, we would be happy to transfer those records to another providerRegards, Meg *C***

Revdex.com: I have reviewed the response made by the business in reference to complaint ID 11424188, and have determined that this does not resolve my complaint For your reference, details of the offer I reviewed appear below As stated in Meg's own response, Dr Klockowski stated the rates would remain the same Same as what, no one knows This ambiguity is what has led to the current situation It was Mom's understanding that the rates would stay the same (as Meg, in her response stated they would) per her conversation with the doctor Except the question that was asked to Meg was would the rates stay the same with or without insurance Meg brought back a response of yes from the doctor In most cases when an employee does not clearly state the circumstances that employee would accept some level of accountability We understand at this point the Dr's office has no interest in be cooperative and Meg is clearly trying to protect herself as opposed to doing what is right Our contention is not what Meg is claiming she said, which we have no doubt she is going to state in writing to be exactly what she should have said Our contention is the paperwork and message from the doctor never indicated there would be different rates without insurance In fact quite to the contrary, even the doctor confirmed they would stay the same At this point, once again, we would like the $in fees returned Regards, [redacted]

I'm writing in response to the complaint that was submitted on May 2, 2016The assigned ID #is [redacted] I received a reply from Thomas that I have not resolved or acknowledge the issues of insurance and ratesThe patient came to our office on March 11, for an Initial Exam and consultationAt that point the patient had insurance which gave her a special PPO feeMom was given payment option on that feeMom called the office on April 11,and first spoke to Sandy then she directed the call to meMom told me she lost her insurance and was probably going to get Cobra and would the fees stay the sameI replied I would have to ask the DrI asked and he replied yes they would stay the same.The Patient next appointment was on May 2, to start treatment, I again verified benefits April 28,and there were none, so I mailed Mom a new payment option without any insurance and with the fees.I can not give a patient a PPO fee if they do not have that insuranceI would never have agreed to give a patient a PPO fee unless they have that insurance, the Mom told us she was going to get cobraThat is why I kept the fee the same.As for the refund on the records, as stated in the previous letter a service was providedand paid forWe will be happy to transfer those records to another provider.Thank you,Meg D.~b-Insurance Clerk

This response, while not accurate to the details of the conversations between the patient/patient's mother and the doctor's office staff, ignores a phone call that took place between 3/and 4/during which Dr Klockowski himself was made aware of a termination in insurance benefits and agreed to honor the original quote regardless of insurance existing It also ignores the circumstances of the 4/appointment where once again, Dr Klockowski and his office staff were made aware that dental insurance was no longer being or going to be carried and still agreed to accept the previously identified rates The bottom line is the doctor and his office staff knew there was no dental insurance and agreed to still accept the rates as previously quoted on two occasions Again, in fairness, we seek a reimbursement of the $in fees paid Meg's commentary that the rates are based on the office's negotiated insurance rates is an issue for them to resolve so their pricing options letter is not misleading.

treatment regular braces for 18 months and upper treatment only for 12 months. We participate with her insurance so we gave her the participating rate. Three weeks later Mom loses her insurance, and states she will obtain cobra Insurance. I recheck on her insurance again on Thursday April 28, 2016 and it was still terminated. I sent out a new payment option to Mom with our normal fees and not including insurance benefits. On May 2, 2016 the patient and Mom had a pre-treatment diagnosis with Dr. The bill was different from the original payment options because of no insurance coverage. I explained to Mom because she didn't have insurance we could not give her the PPO Fees. We were asked for a refund on the money Mom paid for records and explained that a service was provided and that a refund was not going to be given, we would be happy to transfer those records to another provider.
