evaluation, which is most often the case, we are not able to provide exact figures as the physician has not yet evaluated the patient and has yet to determine which, if any, medically necessary tests may be needed for a proper evaluation. We are an Allergy and Immunology office and, as you might imagine, allergy testing is often used to evaluate patients. The need, type, and amount of allergy tests a patient requires is generated by their current and past medical history and determined by the medical provider.When patients seek care in our office our Office Policy regarding copays and deductibles is clearly written and is signed by the patient before services are rendered. I have attached a copy of the office policy for your review. The patient in question did, in fact, sign this policy before any services were provided.Our office participates with numerous different insurance companies with hundreds of different coverage policies. As clearly stated in the Office Policy the patient is responsible for understanding their particular plan, coverage, and benefits.Our Office Policy and practices are consistent with current medical business practices not only in this area but nationwide.In this particular case, the charges were submitted on behalf of the patient for consideration by her insurance company. Her insurance approved and processed all the services provided. We have applied their payment and participation discount to the balance due. The remaining patient balance is the result of deductibles and copays according to her insurance company's policy.Our office did collect the office visit copay at the time of service. As a matter of written policy, that this patient signed, patients are informed that they are responsible for additional copays and deductibles. She was told that we would submit her claim to her insurance company for processing and would let her know if there was any additional patient responsibility.
