October 14, 2015 To Whom It MayConcern: First of all I willstart my response  today by stating  that “[redacted]” has “No Showed” a minimumof 6 times for confirmed appointments at Dr. C[redacted] office in the last 1  1/2 year. But when he has been called from our...

office and told he has dental careto be taken care of, His reply “he is struggling and to busy” .  Thenhe calls the office with a toothache with uncontrollable pain and has to beseen “right” now.On 9/15//2015 patientphoned Dr. C[redacted] office complaining about extreme dental pain in the lowerright area.  Patient had been told “manytimes regarding treatment needed to be done on the tooth he was complainingabout in the lower right area” patient  continued to refused treatment. Now he has pain and he “expects our office tooffer service to this patient immediately when we have a full schedule, he has “NOSHOWED” many times in the pass, He never has PAID his bills in a timely mannernor pays his bills when he says he will, they are paid 2 to 4 months late andonly when I have phoned 3 or 4 time to receive payment. After taking with Dr.C[redacted], we made time in our schedule to see [redacted] today, [redacted] was told that treatmenttoday would be $575.00, this was after Dr. C[redacted] confirmed that this was thesame tooth he had already been told needed treatment and treatment was startedon 9/15/2015, and that payment would be due today and he would have to pay infull today. He then scheduled another appointment for the following day tocomplete treatment and he knew what the cost was and that he would  also be charged for that because of his pastpayment history. He ask for pain medication and he left for the day.On 09/16/2015 [redacted] “NoShowed for a confirmed appointment.  Hegot what he wanted yesterday, he now is out of pain, and he has painmedication. And he is doing the same as he always has before.  He was totally aware he had an appointmentfor 3 hours today in which we could have gave to someone else who really caresand would be here. I phoned [redacted] three times this morning and left 3 messageson his cell phone, but there was no answer and no returned call, you wouldthink this person would at least have the respect to call us. But he got whatthey wanted. It would really bewith respect if [redacted] would tell the truth. [redacted] has never paid Dr. C[redacted] $70,000.00 or over $70,000.00 for dentaltreatment since 2008. What the truth is [redacted] has paid Dr. C[redacted] $20,414.74since 2008.Also, [redacted] isn’tbeing honest about a refund being credited to the credit card that was given toour dental office, we have had the approval codes from the bank and he is alsoaware that he was to bring a check into the office.  [redacted] causes his own problems.

First I have never received any payments for account#[redacted] since 5/28/2015, and that payment was for $100.00. Patient [redacted] was approved for $6,000.00 thru Care Credit on 10/08/2014 and pt. agreeded to make $1,000.00 per month payments on his account which never happened. The balance...

hasn't increased since 11/10/2014 when the patient had a crown prep. Which I can increase the balance and have no problem with finance charges. Patient has not contracted this business. He was to make $1,000.00 dollar per month payments on the approved care credit and that never happened. I just received a letter from [redacted] Bank, patient seems to be concerned about his credit, and that is to late now.  No payment for over a year, he never came into the office to take care of his responsibly.  Yes his account was turned over to collection. And I have talked with the lawyers at the collection agency.
