Review: I was injured at work and was sent to Rockwood clinic and seen [redacted] a PA there for a few months.. On June 26th 2013 she closed my [redacted] claim with out my knowledge. Then on July 9th 2013 (after waiting a week to see her) I went in and talked with her and explained that the case should not have been closed. She referred me to get an MRI which was scheduled for July 24th 2013. On July 11th, I hade to go back to the clinic to fill out the reopening of the claim paper work that had not been given to me on the on the July 9th. I waited 3 weeks longer and had to cancel my MRI (this whole time I have been in pain and taking [redacted] all day) just to get a phone call from [redacted] telling me that the box was checked by [redacted] NOT to REOPEN. I did not ask for the claim to be close I m STILL IN PAIN. I spoke with Rockwood clinic patient rep [redacted] and she told me there was nothing I could do and to go see another provider. ?????I then received a BILL from Rockwood Clinic saying I have to pay for the last visit! I work part time and get 600 a month to try and get by on and I DO NOT OWE THIS... my L & I claim should never have been closed and I was told one thing and L & I told another. I will never pay this to rockwood and am trying to get help with this... can you help me? Good Afternoon,

I am responding to the concern from this patient. Our records indicate the provider of care indicated on 5/30/2013 the L&I claim was going to be closed. A form was completed and given to the patient at that time notifying her of this. We would be happy to release her medical file for this time period with the proper consent from [redacted].

Additionally, the [redacted] sent the provider a form to complete verifying maximum medical improvement 6/3/2013. This was completed and returned to the claim manager on file. [redacted] should be aware of the reason for this from her May visit. (This form was sent to the injured worker, [redacted], the employer and the provider.)

The above took place prior to the visit/charge in July. We are most willing to assist [redacted] with her financial difficulty in paying for this charge. On 9/13/2013 our Business Office sent a financial assistance application. We have not received this back. I will have another sent to [redacted]. Once she completes this and returns the necessary paperwork to our office we will consider her for financial assistance for the balance on her account.

I would recommend that [redacted] contact her claim manager, [redacted] if she has questions relating to the closure and denial of her reopening application.

I hope this information resolves/clarifies the situation for [redacted]. We would like to assist with her financial obligation however, we need the supporting documentation returned to our office.
