Review: As a part of my preventive care visit, a blood draw was done on Dec. 9th. However, claim [redacted] was submitted with a procedure code that is not for a preventive care service (as per the UHC Coding Summary for [redacted]. Consequently, for a service that should have been free, cost sharing has been applied by the insurance company and I have been asked to pay.Desired Settlement: I request [redacted] to review this claim/service and resubmit a correction with the correct procedure code.
