Complaint: [redacted] I am rejecting this response because:See attached rebuttal Sincerely, [redacted] Clearly this is going nowhere, and is a classic case of victim shamingHer response makes me seem like Iam stupidOf course I never expected or thought there was 100% chance of pregnancyThere is no suchthing as 100% anything in life.All I can really say is that the vast majority of the Doctors response is UNTRUE.No we never discussed adoption or sperm donor.No, did they ever tell me I needed a follow up visit, only that I could if I wantedWhich, what would bethe pointI reached out multiple times privately (with no response) about how I felt they had failed me.Just because we had frequent communication, mostly me reaching out to them, doesn’t mean they aregood at it, and did it properly.No she gave me zero info on the “research” sperm test, other than what it was calledNo explanation onwhy it wasn’t mentioned soonerNo explanation of it being in the test phase.As I said previously, I strongly believe the prescriptions should be ordered on a case to case evaluationIunderstand its necessity for some casesMy case however, wasn’tWe had more than enough time toorder meds later (roughly 6-8weeks).Being a good doctor doesn’t mean you don’t make mistakes, you are still humanBeing an excellentdoctor means owning up to them and apologizing when you are wrongI am not questioning yourcredentials I am questioning my particular caseMy particular case that was so poorly handled.It is so easy to say that I am doing this because of the result we got, but that isn’t it at allI was unhappybefore we got the worst news, and it is documented in emailsWhen all you have to use against me issuch a low thing, it really shows just how uncompassionate you truly are, especially at a time whenpeople need compassion

This letter is in response to a letter received from the Revdex.com. We are sorry to hear that *** was unhappy with her care and will address her complaints as brought to our attention in her letter dated 12-11-.(I assume all the proper HIPAA releases from *** or her
husband to discuss his or her treatment/results in detail are in place. If I am incorrect, please do not review the remainder of this letter and contact me immediately.) Her first complaint was for Misdiagnosis. Attached you will find a copy of the patient’s initial History and Physical from her visit to our office on July 26th, As you can see the patient was given the diagnosis of Male factor infertility in a female patient, as highlighted on her history from her first visitHer diagnosis is correct based on her husband’s test results, and she verifies in her complaint that they did IVF due to her husband’s poor quality sperm There was no misdiagnosis. Despite the fact she called this a misdiagnosis, the likely reason for her complaint is that she failed to achieve pregnancy or produce any good quality embryos. This is not a misdiagnosis but a medical reality that only biology controls We understand how disappointed *** and her husband are but we did not misdiagnosis the patient and treated her with appropriate and compassionate care When we initially met ***, we discussed her chances of pregnancy, which she confirms in her letter. Her chances of natural conception were

*** ** *** *** was developed with a swale at the rear of the property line to carry water away from the cul-de-sac and down to the pondThe lot was prepared for construction by adding fill dirt to level the building pad at an acceptable height in order to keep water running away from the final
structure & to meet the road correctlyThe attached photos show this dirt soon after it was addedMajestic Homes has not had a chance to final grade this lot due to weather and the misunderstanding with the clients so it currently sits with the sharp fallMajestic Homes is happily providing *** and *** with the truckloads of dirt as discussed prior to closing, and per ***’s email 4/3/has offered to assist them in having the additional free loads of dirt they received from the neighboring lot incorporated into the final grade Approximately early October when concern was raised about the slope in the back yard in *** ***’s office she told the *** that the yard would have final grade at closing (or as soon as possible weather permitting), but that *** *** would be able to answer this question much more thoroughly as she doesn’t get involved in yards or dirt work*** was in the office at that time, and he stepped into ***’s office where *** told him his concerns, and *** assured him that Majestic Homes would bring in two additional loads of dirt to help ease the final grade, and let him know that there would still be a slopeAt that time *** seemed satisfied with that answer All emails and texts between Majestic Homes employees and *** *** and their representative *** *** have been printed and reviewed, and no where can we find correspondence of the lot being corrected to look similar to their neighbors’ lotMajestic Homes believes any recollection the buyers or their representative have of *** or *** saying the lot would match the neighbors’ is a misunderstanding, and the only guarantee was a final grade to be performed Before signing closing documents *** and *** wrote a ‘punch list’ where they listed everything needing addressed, Majestic Homes addressed these items and followed up with a ‘Final Acceptance Agreement’ where any outstanding work was listedAny comment or concern of the grade of the lot was not listed *** *** Majestic Homes 10355-C Dawson’s Creek Blvd Fort Wayne, IN Cell: 260-797-Office: 260-553-
