*** started her son ***'s treatment on 7-8-Last payment made on his account was on 2-11-The account was turned over to our attorney for collection on 7-29-On 11-11-15, our attorney closed this account because they were
informed she filed for bankruptcy*** contacted our office in April of 2016, and she wanted her son's records transferredI told her I would ask Dr*** if she could return to our officeI convinced Dr*** to let her return since her balance was *** She was seen in our office on 4-26-Dr*** told her due to the face the account had gone to collection, the original contract is now nul and voidHe would continue ***'s treatment if she agreed to paying the original balance, and she would now have to pay an additional fee of *** for a retainer when the braces are removedShe agreed to a verbal contract that she would now owe *** She said she could afford *** She made a payment of *** on X-and 5-24-Patient came in on 6-21-for treatment, and no payment was made at that timePatient had an appointment scheduled on 7-19-16, and cancelled their appointmentHer mother is a patient of ours, and she was seen on 7-19-First thing her mother asked me when she arrived was that her daughter needed a copy of ***'s original contractShe said she did not think it was alright that we were charging her another *** I told her, the contract was null and voidShe said her daughter would be contacting a lawyerI said that this was unfortunateI did give her mother a copy of the contract onlyNo ledger's regarding her payments was givenShe said they were going shopping that afternoon, and she would give it to herI told her mother she could speak to Dr*** if she wanted him to further explainShe said no, that she was not going to discuss her daughter's businessI had not heard from *** until 9-14-She said she read the contract, and did not feel she owed *** I told her I had discussed this with Dr*** He said if she did not want a retainer after braces were removed, we would remove the feeShe said that was fineHis teeth were straight, and she could go anywhere for a retainerI told her Dr*** could not understand why she could not pay her original billShe never contacted us to resolve itI did make the mistake of asking her she did not ask her mother for helpShe got defensive, and she said she could not believe I just said thatShe is an adultShe said it was not her mother's business to know about her bill, and it was an invasion of privacy that I discussed it with her motherShe told me, that she was recording this conversationI did not reply(You are suppose to inform someone before you begin a conversation)She said she was willing to pay the *** She wanted to speak with someone else or Dr*** I told her he would be in tomorrowShe said she would be contacting the Revdex.com and hung up! *** contacted our Valparaiso locationSharon scheduled her an appointment on X-at our Munster office

They have admitted what I saidMy Balance was *** The added on an expense that was already included in my original contractI had an appointment X-XX-XXXX and cancelled but I also called and left a message to have *** *** call me back so I could rescheduleThere was no verbal agreement that I would pay *** because I knew I didn't owe *** My mother coming into an office to retrieve information for my son (whom she has no legal right to) is against the lawHow can someone just walk in and say I need this information and you hand it overThat is a breach in my son and mines privacy.The fact that Mary *** discussed any parts of my bill and informed my mother that my contract was null and void was a breach in privacyShe gave my mother a copy of a contract which had how much I paid for my sons braces, That is a ledger showing paymentMy concern is how someone (including my mother) is able to walk in and just request something and receive itWhere is the security and protection of my privacy? It;s not for anyone to understand, why the *** could not be paid, it was filed in Bankruptcy and I willingly agreed to pay that balance in order to fulfill my contractNowhere on the contract does it say the contract is null and void*** *** has admitted to breaching privacy, offending and discussing my personal business with someone who is not authorized to receive any information about me or my sonI did record *** *** and in Indiana that is absolutely legal as long as one person consents (even if it's the person recording)I received a call from *** from the Valparaiso location and she informed me that Dr*** is willing to remove the *** for the retainer and still *** the original contract which includes a retainerMy balance is *** which will be paid on Sept which is my next appt, however I would like in writing that this is the agreement so there is no further confusion
