Dear Sir/Madam:

I am writing to the Revdex.com to request your direct intervention. Unfortunately, multiple misdeeds by different personnel representing Roosevelt Memorial Park demand I seek help.

All of the following information is truthful. My Mother was buried in Roosevelt Memorial Park last November, 2012. By religious tradition, a “Bronze Marker” may not be placed on her gravesite for at least six months. (Be advised that the entire cost for the “Bronze Marker” was pre-paid in full by my Mother about 50 years ago.)

Approximately five months ago, I received a handwritten note (copy enclosed) from [redacted], Family Service Counselor for Roosevelt Memorial Park in which she stated the following:

“The marker for [redacted] has been ordered.

You will be receiving a sketch in approx.. 3 weeks for approval.

Thank you”.

Be advised that I never received the promised “sketch” from [redacted]. ([redacted]) [redacted] or from any other person representing Roosevelt Memorial Park.

For unknown reasons, I acquired in the mail their formatted “Memorial Notification” dated “6-27-13” a copy of which is attached. That notice specifically proclaimed the following:

“We wish to inform you that the memorial for the late [redacted]

has arrived and please find enclosed a photograph for your information”.That form also stated the following:

“The installation order has been issued to our superintendent and the memorial should be installed within a short period of time”.

I quickly forwarded a letter dated July 17,2013, addressed to [redacted] a copy of which is attached. As you can see, I demanded that the wrong “Bronze Marker” created without my written consent “never be installed.” I also requested prompt receipt of a corrected “sketch”. Unfortunately, those justified requests have never been honored by [redacted] or by any other officer representing Roosevelt Memorial Park.

Because I have not acquired any identified and signed letter advising of precisely what a named authority for Roosevelt Memorial Park intended to do to correct their mistakes, I was forced to forward a “Demand Letter” dated July 29, 2013 by Certified Mail a copy of which is enclosed. (The Certified Mail was successfully received by Roosevelt Memorial Park on July 30, 2013.) For unexplained reasons, no official representing Roosevelt Memorial Park has bothered to forward their signed letter advising of an acceptable resolution.

I would be most grateful if you would contact [redacted] personally in order to achieve his signed letter confirming the wrong “Bronze Marker” has not and will not be implanted in the ground, and that I will promptly acquire a corrected sketch so I may approve or disapprove all imprinted language.

If you have any questions, please forward in an identified and signed form.

I sincerely appreciate your kind offer of positive assistance.

I just received in the mail your signed notice dated October 17, 2013. I read but was not surprised to learn the following:

"you have written to the firm referenced above on previous occasions.

Since we have not received a reply to our requests for a response, a final notice will be sent to the company todays

Be advised that coincidentally I acquired in the mail on the same day a signed letter imprinted with a date of October 18, 2013 from [redacted]. I am enclosing a copy of his communication. I was disappointed but again not startled to read his too brief and obvious non-committal response written exactly as:

"...and respectfully disputes that there has been any sort of 'purposeful negation,' 'interminable non-performance' or 'unwillingness to cooperate" in this circumstance, as this simply is not the case".

Be also informed that on October 16, 2013,1 acquired Certified Mail which included a signed letter dated October 9, 2013 from [redacted]. [redacted] with enclosure.

I am attaching a copy of the October 9, 2013 letter and enclosure from [redacted]. You will be able to easily affirm that particular communication once again affirms **. [redacted] continues to guardedly invent business practices for his own unjustified purposes.

I was appalled and personally offended by most of his remarks in the missive imprinted with a date of October 9, 2013. First, I had no appreciation **. [redacted] suddenly claimed "there have been similar issues previously with the Cemetery's mailing address versus physical address".

For your information, the specified address for Roosevelt Cemetery has been consistently imprinted on their official letterhead and web site. It is most interesting to truthfully proclaim that a large automobile dealership titled as "[redacted]" as per their official web page imprinting an explicit address of "[redacted], PA". That automobile dealership is positioned directly in front of the physical location for Roosevelt Memorial Park. Therefore, the assertion by **. [redacted] "similar issues previously with the Cemetery's mailing address versus physical address" was nonsense!

[redacted] then wrongly declared "there was no deliberate omission of a copy of my July 30th letter". As reported by me specifically printed in my September 11, 2013 as well as other needed letters, a copy of the "July 30th letter" from **. [redacted] was in fact purposefully omitted in his August 22, 2013 missive. (I am enclosing a copy of the August 22 letter.)

It is my steadfast opinion the copy of the letter dated July 30, 2013 was intentionally not included in the August 22, 2013 missive from **. [redacted] because at that time and for approximately seven (7) weeks thereafter that (July 30) communication was not yet created until about October 9, 2013.

I am additionally including a copy of the July 30, 2013 letter deposed from [redacted] (received for the first time on October 16, 2013). It is most curious to see that the copy of that specific letter from **. [redacted] was intentionally imprinted on blank paper. It is also noteworthy that the "cover" letter dated August 22, 2013 transmitted from **. [redacted] in which a copy of the July 30, 2013 was calculatingly not enclosed was also imprinted on a blank page.

I found the remark by [redacted] in the copy of his July 30, 2013 letter written as follows was particularly disgusting:

"...their "sales manager, [redacted] did reach out to me by phone to discuss that there would be a $100 charge for a drawing of the marker before production but was unable to connect with you (me)".

Further, I am at a loss to comprehend why [redacted] and the presently named Sales Manager would ever attempt to contact me by telephone when I was forced to request by letter "no telephone calls" because of annoying unwanted phone contact.

Most importantly, the first notice I received referring to the "sketch" of the Bronze Marker and Border was (as already indicated) forwarded directly from [redacted] through her brief, handwritten note specifically declaring:

"The marker for [redacted] has been ordered.

You will be receiving a sketch in approx. 3 weeks for approval".

As repeatedly truthfully stated in multiple necessary letters:

"I never received that promised sketch from [redacted] or from any other person representing Roosevelt Memorial Park. I was never given any opportunity to approve or disapprove all of the language and writing style for the Bronze Marker."

I was then mandated to repeatedly proclaim:

"Months later however, I acquired a formatted notice dated '6-27-13' absent of a full name, title and penned signature" written as follows:

"We wish to inform you that the memorial for the late [redacted] has arrived and please find enclosed a photograph for your information".

I am unable to comprehend unending obvious deception by the gratuitous invention of an alleged "$100 charge for a drawing of the marker before production".

If the supposed "$100 charge" was factual back in the early Spring months of 2013, then I demand to know specifically how and why [redacted] opportunely omitted that explicit monetary mandate in her handwritten note?

Also if that grotesque command for a "$100 charge" in order to be allowed to preview a "sketch" was allegedly previously accurate, then [redacted], [redacted], [redacted] or someone else in authority should have immediately contacted me in written form explicitly stating that directive.

As repeated in the October 9, 2013 letter offered by [redacted], I am overwhelmingly perplexed of how I could possibly know that there was an alleged "charge of $100 to preview a sketch" before the actual production of the Bronze Marker and Border. (Perhaps I was required to deploy mental telepathy to know of their unwarranted "$100 charge".)

I also ask how many other bereaved family members have been tricked by this kind of specialized entrapment of a supposed commandment of an "$100 charge" in order to be permitted to review explicit language for a Bronze Marker and Border?

Again, it is very sad that **. [redacted] suggested in the recently invented copy of the letter dated July 30, 2013 that "I call Roosevelt Memorial Park to discuss open issues". I again query why he continues to deliberately negate my repeated specific written wish for "no telephone calls?"

**. [redacted] then in the same paragraph offered an "apology that the drawing was not provided as you (I) expected". That particular proclamation is in direct conflict to the brief handwritten promise received from [redacted] (see above).

Please know that I have spoken at length to many close relatives and dear friends who had to "bury" their loved ones. I let them know of the new alleged demand of "$100" by Roosevelt Memorial Park just to be permitted to preview a "sketch" of the Bronze Marker and Border. Everyone strongly reacted with many saying quite loudly "ARE THEY CRAZY"? "We never had to pay an extra $100 to see a drawing"!

Moreover, the remarks by [redacted] referring to my steadfast belief of the recent alleged inability to "Italicize" the last line on the requested Bronze Marker could never be accepted as reasonable.

Furthermore, I noted in his October 9, 2013 letter the referral to my May 13, 2013 notice addressed to [redacted]. In consideration that I had not yet acquired the explicitly promised "sketch," I made the following request which was "I must assume I will be able to preview the inscription before production to affirm its accuracy." Unfortunately, **. [redacted] conveniently omitted in his October 9, 2013 letter that explicit last paragraph which was included in my May 13, 2013 communication.

It is factual I did not acquire in a timely manner a written response as a result of my letter dated May 13, 2013 in which I requested the last line in "Italics".

It is critical that your Agency learn, understand and freely accept the absolute truth that I did not receive the "sketch" of the Bronze Marker and Border for my Mother until June 29, 2013 which was in fact physically stapled to their formatted "Memorial Notification". (For unexplained reasons, [redacted] continues to deny that precise truthful information.)

That particular unsigned notice declared the following:

"We wish to inform you that the memorial for the late [redacted]

has arrived and please find enclosed a photograph for your information

That form also stated the following:

"The installation order has been issued to our superintendent and the memorial should be installed within a short period of time

As already mentioned, I did not retrieve the supposed (original) missive from **. [redacted] soon after July 30, 2013. Therefore, I could not possibly have any knowledge of the presently superfluous incapability to "italicize" the last line in the Bronze Marker and Border.

In my letter dated July 01, 2013 addressed to [redacted], I stated that "I was quite angered to read that specific information. In that communication, I specifically referred to her previously acquired handwritten note 'to send me the sketch in three weeks.'"

I then stated that "according to the formatted notice however, the marker had already been produced". Therefore, I was never given any opportunity to approve or disapprove the sketch.

The final statement in my letter dated July 01, 2013 was "I request that the marker as shown on the photograph not be installed but quickly corrected".

As a result, [redacted] should have quickly reacted by letter to my additional missive dated July 01, 2013 in which I specifically requested "Italics" for the last line only. It was imperative that she or someone else in authority should have immediately submitted a letter advising of their apparent inability to "italicize the last line in the inscription." Sadly, [redacted], [redacted] Lewis, [redacted], [redacted] or someone else representing Roosevelt Memorial Park never transmitted that necessary written notice in a timely manner.

It is essential that I ask again how persons representing a Cemetery could possibly go ahead and unilaterally "produce" a "Bronze Marker and Border" deliberately absent of my explicit written consent?

Be advised again that I have by my signed letter dated October 15, 2013 forwarded directly to [redacted] by Certified Mail to the correct known address (see above) requiring the following:

"...no later than October 20, 2013 the implant of the "Bronze Marker and Border" at the gravesite for my Mother, [redacted].

That particular Bronze Marker and Border must without exception have

the identical design and 'coloring' as the Bronze Marker and Border for my Father,

[redacted] and read precisely as follows:

'[redacted] Wife, Mother and Grandmother 1919 - 2012 Always ready with a good joke'"

I further wrote in my signed letter dated October 15, 2013:

"In addition, Roosevelt Memorial Park must immediately and forever honor the pre-paid 'Perpetual Care' for the gravesites for my Mother and Father without continued, purposeful negation."

Be informed that I did visit Roosevelt Memorial Park this past Monday afternoon. I was surprised to see a sheer cloth covering on what I (then) presume was the Bronze Marker and Border. I attempted to look at the implanted monument. However, upon removing a very small portion at one end I noticed the inscription on the marker appeared "wet." I immediately recovered the entire Bronze Marker and Border. Therefore at this time I have no visual evidence of its accuracy or satisfactoriness.

If however that cloth cover was placed on top of the Bronze Marker and Border in anticipation of an "Unveiling," then I must inform your Office that again my expressed written wishes have been ignored. In my letter dated May 13, 2013,1 specifically wrote: "Be informed that there will not be an Unveiling."

I respectfully request that you or other named authority immediately contacts [redacted] by signed letter only to demand the following:

-Legitimate explanation for placing on top of the Bronze Marker and Border the cloth covering

-Valid clarification for the too tardy receipt of their refund check legally owed to me and -Explicit explanation for how and why Roosevelt Memorial Park continues to endorse unworthy business practices by unending inventions, not promptly responding to valid questions by requested written contact in a timely manner and failure to honor justified requests.

I again request that your Agency forward copies to me of all letters actually sent to and received from [redacted]. [redacted] or other named official representing Roosevelt Memorial Park.

As a courtesy, I am today forwarding a copy of this response to [redacted]. [redacted] at the designated address in anticipation the postal carrier will be able to successfully deliver mail without any alleged further difficulty. I will also include a copy of your official notice dated October 17, 2013.

I must assume **. [redacted] or someone else will address me as the "Executrix" for my Mother's Estate and will freely offer truthful answers to all remaining legitimate queries.
