We regret that a former patient of ours has experienced an inconvenience after discharge As this is the first notice that we received of any concern the patient has had regarding his medication, we have attempted to contact him directly to discuss his concern Unfortunately, we have very little influence over the processes and procedures of other organizations, namely [redacted] Additionally, the medication list for this patient was reviewed by his attending physician and by Administration and no discrepancies or omissions in the medications that were prescribed for this patient, post discharge, were found A very detailed discharge plan is carried out for every patient who receives care at Rosewood Post-Acute Rehab In many instances the medications that were prescribed while the patient resided at Rosewood Post-Acute Rehab will not be refilled by the his/her attending physician post discharge and the patient must seek a new prescription from their primary care physician, if they wish to do so The reason for this policy is that some medications require special attention as it relates to administration of those medications, and must be overseen by a licensed practitioner, for the patient's own safety Additionally, the California Department of Health and CMS also have very strict regulations and guidelines as it relates to post discharge prescriptions of certain types of medication However, other medications that do not require such scrutiny can and are prescribed for the patient post discharge and may be re-filled at their local pharmacy; as was the case with Mr [redacted] .Should you have any other questions regarding this matter, please feel free to contact us directly Thank you?

I am rejecting this response because: the form was improperly filled out. And when I contacted Rosewood I was told to contact my primary doctor.

Review: Upon discharge I was given a Discharge Medication Prescriptions form and told I could get refills with it. I took it over to [redacted]'s pharmacy was told that they couldn't give me refills because the form said I had received them all. I have not.Desired Settlement: Fix the problem.
