CID #[redacted] We are sorry about the patient’s misunderstanding of the timing and contents of the contact lenses shipments.  Ms. [redacted] was shipped the remainder of her order.  The complete one-year supply was delivered mid-December 2016.

Initial Business Response /* (1000, 5, 2015/08/11) */
Thank you for the opportunity to address Mr. [redacted]'s concerns. We have reached out to our client to our client to get their explanation of the services rendered. Mr. [redacted] paid $484.56 cash for his frames and lenses. This is separate...

and unrelated to the insurance claim and remaining balance which derived from the eye examination. The amount billed to the insurance company was $623.00. The insurance coverage of Mr. [redacted]'s insurance policy only covered $311.92 of the charges for this procedure. At the time, Mr. [redacted]'s deductible under this insurance policy had not been met. Therefore, the balance of $311.92 was applied to the deductible of his insurance coverage and he is personally responsible for this amount.
Due to the confusion around this claim, we will delete the collection item from his credit report. But, the balance of $311.92 will remain due. We will arrange for an itemized statement and copy of his Explanation of Benefits from the insurance company to be sent to him. I would recommend Mr. [redacted] do two things; first, contact his insurance company and appeal the claim for them to cover the entire cost of the Eye examination. Second, I would continue to work with the provider to see if they will provide assistance with this appeal and/or provide any other consideration to the situation. We are unable to provide Mr. [redacted] a letter stating this was a result of a billing error. We did not bill the claim, our client did, and we don't believe there was an error.
Again, we will delete the collection item on Mr. [redacted]'s credit report and send him a letter confirming the deletion. We will arrange for the bill and Explanation of Benefits to be mailed to him. We will discontinue our collection efforts, but until our client informs us differently, the balance remains due and we will accommodate Mr. [redacted] with a manageable payment arrangement to fit his situation.

Initial Consumer Rebuttal /* (3000, 12, 2015/08/27) */
Please be advised that I just returned on August 24, 2015 from a 2-week vacation with my Family. Therefore, I was UNABLE to review the Revdex.com Response from Mr. [redacted] President of Merchants Credit.
Whilst I respectfully appreciated Mr. [redacted] decision to delete the Collections Account of $311.00 from my Credit Report, I am NOT satisfied with the rest of the explanation provided.
My point is simple and very straight forward: "Why should a simple Eye Examination cost and/or be billed for $622.00? This is clearly MEDICAL OVERCHARGING and/or MEDICAL BILLING FRAUD. Especially so, when 99.9% of all of Chicago-based Opticians charge between $40-$80! Indeed, I believe I was charged between $40-$80 by the Optician/Client of Merchants Credit in 2011, when I first visited this same Optician for the same so-called "Eye Examination". This fact clearly explains WHY Mr. [redacted] Optician client has REPEATEDLY REFUSED to provide me with an Itemized Biliing.
For the record, I HAVE NOT received any of the Correspondence or Itemized Billing that Mr. [redacted] President of Merchants Credit promised to send to me in its Revdex.com Response, which was filed with Revdex.com more than 14 days ago. May I respectfully suggest that the Itemized medical bill and/or any other Correspondence be sent to me by CERTIFIED MAIL or any other mail that requires the Recipient's Signature. And that ALL COPIES of such Correspondence be COPIED to Revdex.com ADVOCATE on this case Thank for your kind and PROMPT assistance in this vexed matter.

This patient was seen by ophthalmologist Doctor [redacted] for his visit with us in March, 2014, NOT an Optician, as he states in his complaint. During the course of the examination Dr. [redacted] was concerned about the possibility of Glaucoma with this patient and performed medical-evidence based testing to rule-out glaucoma. The result of that testing led her to wish to monitor the patient at future visits. The fees associated with this testing were appropriate and justified. It is clear the patient does not understand what he received, nor does he want to pay his bill.
