To Whom It May Concern, I am writing to address Mrs***’s complaint. This patient received a routine comprehensive intake which does not just include an MD visit, but also: 1) pvisit assessment & form processing (support staff and MD minutes), 2) visit
with MD (minutes) and 3) a written post visit summary and Initial Treatment Plan (MD-min + support staff )An initial visit with Dr*** is never just a hour contact. Mrs*** had an initial consultation and was given an extensive intake and consent forms to be completed prior to her arrival. A few of the forms are attached, the rest, I’m not able to attach blank. The supplements recommended are purchased at the patients accord either in our office or outside the office through a vendor of the patients choice. The treatment plan is often provided a day or so later with the complete list of testing recommended and any other recommendations. ***, Dr***’s assistant, provides this with an explanation and coordinates sending any test kits to the patients at that time. The laboratory blood tests that were crossed off were declined by the patient because she claimed to already have had them tested at another office. The laboratory for the saliva kit will only bill if they are able to process the sample. If the patient did not collect or store the samples properly, it will not be run. In an effort to offer good customer service, we are happy to offer Mrs*** a refund of her office visit and any internal laboratory administration fees that were charged. We are not able to refund her any funds that were billed from the lab itself. We do offer an apology for her dissatisfaction. Please feel free to contact me with any further questions.
