Sir/Madam: We can assure you that [redacted] was not sent to collections without warrant On March 30, [redacted] called the center to cancel and spoke to our Sales Manager, Russ [redacted] During this conversation Russ reviewed the requirements to cancel a membership, as clearly stated on the back of the membership contract Russ told [redacted] that she needs to send notice via certified mail if she was unable to come inRuss also called [redacted] back later that day and sent a follow up email in an effort to address her next billing, but the phone calls and emails were left unreturned In addition to Russ’ efforts, phone calls and emails were sent by our Member Service team, and those too were left unanswered [redacted] never cancelled her membership as instructed, and never responded to the calls and emails Since we had no way to reconcile the account, [redacted] account was sent to collections in May Just last week we received payment of $245.85, but the outstanding balance was $ We appreciate the effort to bring the account balance down In an effort to resolve the matter, and hopefully have [redacted] return as a member in the future, we have cancelled her membership and applied a credit to her account for the remaining balance In addition, we have removed the account from collections and we consider the matter closed Please let us know if you should require anything further Yours in good health, Steve L [redacted] General Manager

Our General Manager reached out to [redacted]. While we do have an executed membership agreement and it appears that she has had a nurse appointment and training orientation. We understand her hardship and we will refund her membership in full ($212).
