[redacted] my son went to the clinic for X-ray service on 12-30-It was a preventative diagnostic serviceIt was not a urgent care visit We received a statement (patient a/c XXXXXX [redacted] date of 1-19-in the amount of $regarding [redacted] visit on 12-30-It was coded as urgent care visitI did not agreeThe business was listed as Doc Imaging Service on United Healthcare website as in-network imaging providerI went to the address and was not aware of its urgent care serviceThis visit was referred by [redacted] 's doctor because she wanted to make sure he does not have any abnormality regarding his back (preventative service)With my insurance coverage on preventative service , it should be covered without co-pay or deductiblesI am asking the clinic to re-issue the bill to my insurance conpany using preventative service code

refund was suppose to be given and never was received Had a procedure done in June of and paid in full for sessions, a total of After one session I was unhappy and didn't go backI called for a refund a couple months ago and they said they were unable to give a full refund because of the time frameI wasn't told of a time frame in the beginningThey came back and agreed to give us back and free sessionsWe agreed to the lesser amount and about a week later still hadn't received a refundWhen we called back they told us they couldn't do it and said they can now only give a refund of We have agreed to that but it has been weeks and we still haven't received that eitherNow we are asking for your help!

The facility was dirty and the staff was unprofessional in parading patients through the office area where patient records were maintained

I have been a patient at the Dayton Outpatient since *** *** PA was my predominate provider and she practiced under Dr*** ***Randomly, the office would require me to submit a urine drug screen which was part of my pain management and involved receiving a narcotic pain medicineMy insurance has refused to pay for the required urine drug screening Due to a loss of employment which led to financial hardship, I could not afford to pay the urine drug screeningI spoke to Dr.*** about it and he informed me not to worryHe told me that if my insurance would not pay for the drug screening and that he would write off the costAfter this, I kept receiving bills that were difficult to understand so I spoke to the billing department on numerous occasions
On 8/17/I discovered that my provider was no longer working at the facilityT
