The patient was seen in our office for a check upA 3D cone beam radiograph was taken at that appointmentWhen it read, we discovered a narrowed airwayThis would indicate a tendency to patient having sleep apneaWe then discussed her sleep habits and patterns and gave her the option of returning for a full evaluation and consultation for referral to have a home sleep study administeredWe also told her since sleep apnea is a métical diagnosis all billing would be done through her medical insuranceShe was asked to bring her medical insurance card at the next appointment,We were and are always transparent about the facts and treatment including our relationship with the company that administers the "unattended home sleep study" which has staff doctors that read and interpret the results, since we are not experienced in MEDICAL INSURANCE CLAIMS, they also handle any and all of our MEDICAL INSURANCE coding and billingPatients are always told since we are dentists we are not in any medical insurance networksIt is always the patient's' responsibility to know their own insurance coverageWe provide billing to their insurance company and direct pay to us as a courtesy.During her apnea exam and consultation, we performed a full evaluation and reviewed multiple pages of sleep apnea questionnaires, meant to further evaluate her issues for the purposes of deciding the necessity for a sleep studyAfter a full and thorough evaluation,(which we have all the documents) it was determined a sleep study would be prescribed and a referral was made to Sleep OptimaWe then reviewed, on a model, how the device should be won, and what it would recordWe explained that Sleep Optima would contact her in the following days to arrange deposit and shippingShe was charged $for this visitSome time during this appointment, Ms [redacted] gave us her medical insurance card, and we scanned it to her account.After being contacted by Sleep Optima Ms [redacted] declined the studyShe then forgot all of the previous appointments, discussions, and questionnaires pertaining to our diagnosis along with the discussions about medical insurance coverage, Sleep Optima informed our office that her insurance would not render any payment due to having a high deductible, any coverage for our service was applied to her deductible, We offered to make a courtesy adjustment to a portion of her balance, which she gladly acceptedAfter this she refused to pay her balance and continued to harass tiny staffShe then accused our office of scamming herShe was then under the misbelief she could siow pay and not be considered delinquent, while every statement snereceives says bill must be paid in full or interest may be assessedShe then complained about this and we told her we would now rescind our courtesy adjustment and she would be responsible for the rent life balance.Sleep Optima was contacted by our office and a copy of the for In was requestedWe received that form and it was faxed to Ms [redacted] at the fax number she provided on May 16, 2016.With respect to forwarding records it is our office policy not to forward the records of any patient that has an outstanding unpaid balanceAfter she reported us to the Ohio State Dental Board (OSDB) we received a call saying we must release radiographs, which we then CompiledFor the rest of her complaint sent to the OSDB and with the Ohio Dental Association, she was breech to her standard of care, and they will not address any disagreement
