On 5/18/went to my doctor for a WELLNESS Visit
my visit is code wrong and I am being wrongfully billed
On 05/18/I saw my family Doctor Dennis Sumsiksi
he has been my family doctor for my parents and for the last yrsuntil my mother passed *** last December
I Enter the office and told the nurse that this is a WELLNESS visit per my UMR insurance company health care planI get free visit per yearDoc ask me how I was doing we talked about mom I told him it was hard being an only child and that I need all my meds refilled including XanaxHe told me I had gain weight and needed blood work
That was the wellness visit if I cant get meds refilled and talk to the doc what is the wellness visit
I know that what they get from me would more then insurance but

Received bill from them for $which was paid at their website according to instructions on billSite gave a Ref# for paymentThey couldn't find paymentPaid again at website on X-with another Ref# given for for proof of second paymentAgain couldn't find itNeither had been debited from our bank accountFaxed copy of receipt to *** at Billing 2-2-Talked with indiv at contact # ref our good faith efforts to pay according to their billing instr at their websiteWas told they would try to figure it outThey subsequently turned it over to collections facility (H&R in Cedar Rapids with # for confirmation) who contacted me on X-and which I paidThey obviously have problems on website which they can't figure out and then penalized me with a collections pursuitOffensive to me and my husband to have collections involved especially whenever we were not at faultWe pay our bills and made good faith efforts twice to pay this oneThey have very poor business pr
