*** was called several times with an apology for the December charges for ***, and explained to her that it was a system problem and that a credit would be issued for the overcharges.*** misunderstood the charges for December medications and felt that she should receive a total credit
of ($450.00) for the medications billed on December 31st and the overcharges of the medication in December. It was explained to *** that the medications were sent and received twice in December due to the timing of when the medications have to be taken within a day period of time, so the patient did receive the medications twice in December (this was explained to *** in February and March). We explained to her that the overcharge would be credited to ***'s account in the amount of $(credit issued in February). Also, attached is the revised statement and apology letter to ***. ***'s credit card was charged for the December and January medications, and a credit was issued in the amount of $for the December medications. We did not charge ***'s visa for February, but instead applied part of the credit to the balance due in February in the amount of $164.44, and issued a refund to ***'s credit card in the amount of (see attached statement). As of February, *** has a zero balance, which does not included March medications.We did apologize to *** several times by phone and by letter with an explanation for the December charges, and inform her of the credit that would be issued. *** is continuing to receive medications from Saliba's Extended Care in March
