I am writing to inform you that as the owners of Salon Images that the problem
concerning Mrs*** was resolved on Thursday Oct22, (case # XXXXXXXX)
We met with
Mrs*** and examined the injury inflicted upon her hairline
The operator Diane Chandler-Walton is an independent contractor and we were
unaware of the incident until receiving your letter describing the problem
After inspecting her skin it showed irritation caused by hydrogen peroxide
The violet coloration had disappeared after shampooing a few times, so this
corrected itself
We refunded the remaining monies
The operator Diane Chandler-Walton will be re-educated in chemical use and client safety
as we have never had a problem like this in the years of operation
Please inform us if she fails to contact you with an explanation of the incident, this was her
responsibility as an independent operatorWe seriously take the performance of any person
working under our mantle
