Review: I purchased a lipo laser and B12 treatments to help with cellulite reduction. There was no doctor on staff providing the treatment at the Murrysville Location and there was always someone new and different trying to figure out how to do the treatment. I called to complain and get my money back. I was called back once. I left several messages and obviously they do not want to address the unprofessional-ism and un-educated staff in Murrysville, PA and they do not want to credit me back my money for this fraudulent treatment. Such a waste of money and a fraudulent experience. I was also burned by the laser as well...probably because a non medical person applied the laser paddles.Desired Settlement: I already filed a dispute with PNC Bank the card I used for this garbage.

Review: I singed up for their weight loss program. Less than a week later I went to the office and explained that the system would not work for me and canceled. I was billed for the full amount and he not received the credit. This has been almost 2 months in the waiting. They will not return my calls or emails. Once they did and said it was credited but it was not.Desired Settlement: I want refunded ASAP.

Review: I went for an interview for a weight loss program. All information ( I found out later was not disclosed). I paid the fee( $1400.00) and started the program. Had blood work, saw the doctor and was given 23 syringes, no instruction or information on how to use them. I was given a booklet that day and in it , was described everything to do. I was unhappy with what was required, and said I wanted a refund. I was promised a refund if I wasn't happy. I waited several weeks and no refund, so I contacted them again, they talked me into trying the program again. I did and after 10 days said I could not continue. They said I would recieve a refund, I still have not. This started April 8th and it is now August 7th and still no refund. I have sent texts and am told she is working on it. They have had my money long enough.Desired Settlement: I would like a full refund. If I had been given all the information when I had the interview, I never would have paid for the program and started it. I was never able to complete the program, I did lose s few pounds, but it was so restrictive and I felt unhealthy, I could not continue.I feel they need to be honest and open about it BEFORE they take your money.

The patient that issued the complaint against the weight loss program at our [redacted] Wellness location purchased a 14 week fast track weight loss program, on 4-8-2013, after the program protocol and it's average results were explained to her by our Wellness Counselor, [redacted]. The patient read and initialed each page of the Patient Informed Consent and signed the final page that she understood the diet protocol and the Mandatory Adherence to the strict diet regimen; shown in Attachment 5.

Our refund and cancellation policy was explained to the patient, by [redacted] on 4-8-2013, and the patient initialed and signed that she understood each of the bullets. Attachment 3 is the copy of the Scott Medical Health Center service agreement signed by the patient that issued the complaint. The patient then paid for the 14 week fast track weight loss program.

The patient was given an initial exam by a licensed MD at which time he again reviewed the Patient Informed Consent for the protocol and initialed each page that she understood. The MD determined that as long as her diet panel came back without concerns that she would be healthy enough to proceed, the MD then issued a lab requisition for a 12 hour fasting blood draw.

On 4-24-13 the patient had a follow up visit with both the Wellness Counselor, [redacted], and the licensed MD. The patient was shown how to perform the self- administered injections of Human Chorionic Gonodotropin and given a copy of the Self Administered Injection Sheet, Attachment 1. The patient initialed her understanding of all requirements of self-administered injections on the Injection Release Form, Attachment 2. The patient saw the licensed MD who reviewed her lab results, completed a physical exam and deemed it would be safe for her to begin the strict protocol with the Human Chorionic Gonodotropin injections. The MD issued 23 of the 45 injections purchased in the weight loss package. The patient had a follow up visit with the MD on 5-1-13, no concerns or bad reactions noted in chart note by the MD. On 6-26-13 the patient was seen by the licensed MD and issued the second half, 22, of the 45 injections that she purchased with her weight loss program.

The patient in complaint ID [redacted] requested a refund on 7-23-2013 via a text message sent to the Wellness Counselor at our [redacted] Wellness location, [redacted]. A refund statement, Attachment

4, was completed and submitted for approval. In accordance with our Service Agreement, initialed by the patient, the refund was calculated based on the services and products received and the refund was issued within the stated 30 day time period. The balance due to the patient in complaint # 9657038 was mailed on 8-20-2013 less than a full 30 days from the refund request.

I have included copies of the following:

Attachment 1: Self Administered Injection Sheet; This sheet gives complete instruction on how to perform self-administered injections for the patient to have on hand just in case they can't remember what they were taught.

Attachment 2: Injection Release Form; This form is signed and initialed acknowledgment by the patient that they have received full instruction and have a complete understanding of all aspects of selfadministered injections.

Attachment 3: Scott Medical Health Center Service Agreement; Tis agreement is explained bullet by bullet to the patient and they initial that they understand.

Bullet 4 explains the administrative fee of $200 and how the refund will be calculated Bullet 5 explains that any medication or injections taken home are not refundable Bullet 9 explains that Human Chorionic Gonodotropin, hcg, is never refundable once removed from the providers office.

Attachment 4: Refund Statement; This statement is a calculation of all services performed and th total $ amount of all services rendered, administrative fee and final refund due.

Attachment 5: Patient Informed Consent; This form is a detailed explanation of the use of Human Chorionic Gonodatropin as a treatment for weight reduction, all possible side effects, and the mandatory adherence to the diet protocol. This form was initialed by the patient that filed the complaint at the bottom of each page and verified b the MD, witnessed by the Wellness Counselor, [redacted].

Attachment 6; Refund check; This is a copy of the check stub from the refund check issued and mailed to the patient on 8-20-13.

THEY TOLD ME THAT I NEED TO COME IN FOR A CONSULANTION. I TOLD THEM I DO NOT HAVE THE TIME TO DO THAT. THEY HAD SOMEONE CALL ME BACK AND ENSURE ME THAT I NEEDED TO COME IN TO TALK TO SOMEONE IF I WANTED TO GET ANY INFORMATION. I REPEATLY TOLD THEM THAT I DO NOT HAVE THE TIME AND I WOULD HAVE TO CATCH A BUS THERE. THEY TOLD ME THAT THEY GUARNTEE RESULTS!!!!SO I SCHEDULED A CONSULTAION. WHEN I ARRIVED, I TOLD THE WOMAN OF THE OFFER I HAD FROM [redacted] AND THAT I WANTED THAT 3 SESSION OFFER. SHE TOLD ME THAT, IF I WANTED GUARENTEED RESULTS, I HAD TO GET AT LEAST 18 TREAMENTS FOR A TOTAL OF $1200. I TOLD HER I COULD NOT AFFORD THAT AND GETTING THEIR WOULD BE INCONVIENET, AND I WANTED JUST THE 3 SESSIONS. SHE TOLD ME I WOULD NOT GET ANY RESULTS WITH JUST THE 3 SESSIONS AND I WOULD NEED THE 18 SESSIONS TO GET RESULTS THAT I WANTED. I AGAIN TOLD HER I

[redacted] Attn: [redacted]Dear [redacted],I am writing in response to the letter I received from the Revdex.com on 2-26-15 regarding a complaint that was filed by [redacted] with your office.I received a complaint from [redacted] in writing on December 19,2014. I responded to this complaint in writing on December 24,2014 explaining the Laser Contouring Service Agreement that she signed specifically stated that there were no guarantees. (A copy of this Agreement is attached) I did confirm with her that our office had a client pledge that I would gladly extend to her, but she was not interested. A copy of the letter I sent to her is attached.[redacted] called me directly on January 21, 2015 to complain again. I spoke to her at length and explained that there cannot possibly be guarantees on Lipo Laser and that I would gladly give her additional services and she refused. The director also responded in writing to this complaint on January 22,2015.
